目的:观察高压氧(HBO)对茶碱药代动力学的影响。方法:6名健康成年人在正常状态和0.25MPa(2.5ATA)HBO状态下,分别单剂量空腹口服氨茶碱片300mg,给药后抽取静脉血,应用紫外分光光度法测定血药浓度。结果:HBO状态下的血药浓度较正常状态时偏低,所得药代动力学参数经配对t检验表明,两种状态下的消除半衰期和清除率具有显著性差异(P<0.05),但对其它药代动力学参数无显著性影响(P>0.05)。结论:在0.25MPa(2.5ATA)HBO状态下吸纯氧,可使体内氧浓度较正常状态高约12.5倍,从而牙致细胞色素P-450系统对茶碱的代谢加快,清除率增高。临床上使用氨茶碱合并HBO治疗时,应适时按个体化调整剂量,以控制其血药浓度,防止不良后果的产生。
Objective: To observe the effects of hyperbaric oxygen (HBO) on pharmacokinetics of theophylline. Methods: Six healthy adults under the normal condition and 0.25MPa (2.5ATA) HBO state were given single dose of fasting aminophylline tablets 300mg, after administration of venous blood samples were extracted, UV spectrophotometry determination of plasma concentration . Results: The plasma concentration of HBO was lower than that of the normal state. The pharmacokinetic parameters obtained by paired t-test showed that there was significant difference (P <0.05) between elimination half-life and clearance rate But no significant effect on other pharmacokinetic parameters (P> 0.05). Conclusion: Oxygen inhalation in 0.25MPa (2.5ATA) HBO can make the body oxygen concentration about 12.5 times higher than normal, and the metabolism of theophylline is accelerated and cleared by the dental cytochrome P-450 system Rate increased. Clinical use of aminophylline combined with HBO treatment should be timely and individually adjusted dose to control its plasma concentration to prevent the adverse consequences.
