目的:了解基层医疗卫生机构门诊处方用药的现状,促进合理用药。方法:以《处方管理办法》、《中华人民共和国药典·临床用药须知》、《新编药物学》等为依据,以《医院处方点评管理规范(试行)》为标准,随机抽取重庆市某区基层医疗卫生机构2010年7月-2011年7月的463张门诊处方进行点评,判断不合理处方并进行归类分析。结果:存在每张处方用药品种数高、抗菌药物使用率及注射剂使用率高、国家基本药物使用率较低等情况。195张不合理处方(497项)中,不规范处方共174项,占总数的35.0%;药品不适宜处方共262项,占总数的52.7%;超常处方共61项,占总数的12.3%。结论:建议基层医疗卫生机构重视信息化建设,加强相关知识培训,加强处方审核,完善相关制度。
Objective: To understand the current situation of outpatient prescriptions in primary health care institutions and to promote rational drug use. Methods: Based on the “prescription management approach”, “Pharmacopoeia of the People’s Republic of China”, “New Pharmacology” and so on, and based on the “Management Criteria of Hospital Prescription Reviews (for Trial Implementation)”, a district of Chongqing 463 outpatient prescriptions of primary medical and health institutions from July 2010 to July 2011 were reviewed to determine the unreasonable prescriptions and carry on the classification analysis. Results: There was a high number of prescription drugs per prescription, the usage rate of antibiotics and injections was high, and the utilization rate of national essential drugs was low. Of the 195 unreasonable prescriptions (497 items), 174 were non-standard prescriptions, accounting for 35.0% of the total number of prescriptions; 262 were unfit for prescriptions, accounting for 52.7% of the total number of prescriptions; 61 were overnormal prescriptions, accounting for 12.3% of the total. Conclusion: It is suggested that primary health care institutions should attach importance to information construction, strengthen relevant knowledge training, strengthen prescription review and perfect related systems.
