临床上视神经和视网膜神经纤维层与视野测定之间细致的相互关系是对青光眼患者评价的一个重要方面。神经纤维层及其在视网膜局部投射到视神经乳头的解剖结构形成青光眼中结构和功能之间空间关系的基础。临床经验以及结构和功能的时间关系上表明:视神经及其周围神经纤维层的可见改变可发生在视野中暗点被测得之前。视神经和视野的定量临床研究支持这一概念,并为青光眼视神经损害的发病机理研究提供了进一步的线索。
The detailed interrelationship between clinical optic nerve and retinal nerve fiber layers and visual field determination is an important aspect of glaucoma patients’ evaluation. The nerve fiber layer and its anatomical structures that project locally into the optic nerve head in the retina form the basis of the spatial relationship between structure and function in glaucoma. The clinical experience as well as the temporal relationship between structure and function suggests that visible changes in the optic nerve and its peripheral nerve fiber layers can occur before the dark spot in the field of view is measured. Quantitative clinical studies of optic nerve and visual field support this concept and provide further clues for the pathogenesis of optic nerve damage in glaucoma.
