目的观察甲状腺全切除术中应用精细被膜解剖技术对甲状腺癌患者甲状旁腺及喉返神经的影响。方法选择2008年7月至2014年7月广州市番禺区市桥医院收治的120例甲状腺癌患者,均行甲状腺全切术。其中将使用精细被膜解剖技术行甲状腺全切术的患者作为研究组,共41例;将使用传统方法行甲状腺全切术的患者作为对照组,共79例。比较两组甲状旁腺及喉返神经相关并发症。结果研究组暂时性喉返神经麻痹、喉返神经永久性损伤、暂时性甲状旁腺功能低下症、永久性甲状旁腺功能低下症发生率均明显低于对照组(P<0.05)。结论将精细被膜解剖技术应用于行甲状腺全切除术能够减少喉返神经、甲状旁腺损伤的发生率,值得临床推广应用。
Objective To observe the effects of fine capsule anatomy on the parathyroid gland and recurrent laryngeal nerve in thyroid cancer patients during total thyroidectomy. Methods From January 2008 to July 2014, 120 patients with thyroid cancer admitted to Shiqiao Hospital of Panyu District of Guangzhou City underwent total thyroidectomy. Among them, 41 patients underwent total thyroidectomy with a fine capsule anatomy as a study group, and 79 patients underwent conventional total thyroidectomy as a control group. The complications of parathyroid glands and recurrent laryngeal nerves were compared between the two groups. Results The incidences of temporary recurrent laryngeal nerve paralysis, permanent recurrent laryngeal nerve damage, transient hypoparathyroidism, and permanent hypoparathyroidism were significantly lower in the study group than in the control group (P<0.05). Conclusion The application of fine capsule anatomy technique in total thyroidectomy can reduce the incidence of recurrent laryngeal nerve and parathyroid injury. It is worthy of clinical application.
