目的:探讨二维超声及彩色多普勒血流显像对甲状腺腺瘤和结节性甲状腺肿的鉴别诊断价值。方法:分析经手术后病理检查证实的52例甲状腺腺瘤患者的54个结节和78例结节性甲状腺肿患者的102个结节的声像图,对2组疾病的二维声图像与彩色多普勒血流显像图像特点进行比较分析。结果:2组疾病在发病年龄、性别、病灶部位、结节大小、结节退行性改变(钙化、囊性变或者出血)等方面差异均无统计学意义(P>0.05);在结节数目,结节形状,结节内实质回声,结节边界(晕环、包膜),结节内部、周边血流信号等方面差异均有统计学意义(P<0.05)。结论:甲状腺腺瘤和结节性甲状腺肿二者的病理基础不同,构成了超声图像上不同的特征,可作为甲状腺腺瘤和结节性甲状腺肿的超声鉴别诊断要点。
Objective: To investigate the value of two-dimensional ultrasonography and color Doppler flow imaging in the differential diagnosis of thyroid adenoma and nodular goiter. Methods: The sonography of 54 nodules in 52 patients with thyroid adenoma and 102 nodules in 78 patients with nodular goiter proved by pathological examination was analyzed. The two-dimensional acoustic images Color Doppler flow imaging features of comparative analysis. Results: There were no significant differences in age, gender, lesion location, nodule size, nodular degeneration (calcification, cystic degeneration or hemorrhage) between the two groups (P> 0.05) , Nodular shape, substantial echo in nodules, border of nodules (halo, envelope), internal nodules and peripheral blood flow signals were all statistically significant (P <0.05). Conclusions: The pathological basis of both thyroid adenoma and nodular goiter are different, and constitute different characteristics of ultrasound images, which can be used as the key points in the differential diagnosis of thyroid adenoma and nodular goiter.
