目的:探讨尿酸水平与缺血性脑卒中严重程度及预后的相关性。方法:选取2017年1月至2018年10月哈尔滨医科大学附属第二医院收治的210例缺血性脑卒中患者作为缺血性脑卒中组,选取同期健康体检的210例健康者作为健康对照组;缺血性脑卒中患者根据入院时美国国立卫生研究院卒中量表(National Institute of Health Stroke Scale,NIHSS)评分分为轻型卒中组和中-重型卒中组;参照改良Rankin量表(the modified Rankin Scale,mRS)分为预后良好组和预后不良组。比较各组临床资料及实验室结果,探究缺血性脑卒中发生及预后的危险因素。结果:缺血性脑卒中患者尿酸水平明显高于健康对照组[(321.10±75.07) μmol/L比(290.10±68.43) μmol/L,n P<0.05];缺血性脑卒中发生风险随着尿酸水平的升高而增加;中-重型缺血性脑卒中患者的尿酸水平均高于轻型脑卒中患者[(338.60±76.09) μmol/L比(298.67±67.77) μmol/L,n P<0.05];预后不良组尿酸水平明显高于预后良好组[(363.44±82.53) μmol/L比(301.65±59.61) μmol/L,n P<0.05],尿酸是缺血性脑卒中患者预后的危险因素[n OR=2.013,95%n CI:(2.007~2.019)]。n 结论:尿酸是缺血性脑卒中发生及预后的危险因素,其水平在不同严重程度缺血性脑卒中患者间具有差异性。“,”Objective:To investigate the relationships of uric acid level with the severity and the prognosis of ischemic stroke.Methods:A total of 210 patients with ischemic stroke admitted to the Second Affiliated Hospital of Harbin Medical University from January 2017 to October 2018 were selected as ischemic stroke group, 210 healthy people who underwent physical examination during the same period were selected as the healthy control group.These patients were divided into mild ischemic stroke group and moderate-severe ischemic stroke group according to their National Institutes of Health Stroke Scale (NIHSS) and were divided into good prognosis and poor prognosis according to the modified Rankin Scale (mRS). The baseline data and laboratory examination results were compared between groups, to explore the risk factors for the development and prognosis of ischemic stroke.Results:The uric acid level of ischemic stroke group was significantly higher than that of the healthy control group[(321.10±75.07) μmol/L vs (290.10±68.43) μmol/L, n P<0.05]. The risk of ischemic stroke increased with uric acid level.Also, uric acid level of moderate-severe ischemic stroke was significantly higher than that of mild ischemic stroke[(338.60±76.09) μmol/L vs (298.67±67.77) μmol/L,n P<0.05]. Uric acid level in the group with poor prognosis was significantly higher than that in the group with good prognosis[(363.44±82.53) μmol/L vs (301.65±59.61) μmol/L,n P<0.05]. Uric acid was a risk factor for the prognosis of ischemic stroke[n OR=2.013, 95%n CI: (2.007~2.019)].n Conclusions:Uric acid is a risk factor for the development and prognosis of ischemic stroke, and its level can predict the severity of ischemic stroke.
