目的探讨肝细胞癌(HCC)瘤体周围供血动脉的时间平均血流速度(TVmean)与瘤体内微血管密度(MVD)的相关性,以及TVmean与瘤体细胞分化程度的关系,确定TVmean术前无创性估价HCC生物特性的价值。方法术前超声检测45例HCC患者瘤体周围供血动脉的TVmean,超声检测45例正常人肝组织内细小动脉的TVmean。观察术后HCC组织常规病理染色的细胞分化程度、免疫组织化学染色计数MVD,TVmean与MVD行直线相关分析。结果HCC瘤体动脉TVmean与MVD呈直线相关,相关系数r=0.794,回归方程为y=18.2764+1.5544x。依据TVmean≥21.3cm/s计算低分化HCC的阳性预测值为97.4%,阴性预测值为87.5%;TVmean<21.3cm计算高分化HCC的阳性预测值为88.5%,阴性预测值为97.3%;依据TVmean计算HCC细胞分化程度的可信度达89.5%。结论HCC瘤体动脉的TVmean可于术前估价瘤体的生长状态、细胞的恶性程度,为分析患者预后提供有用的依据。
Objective To investigate the correlation between time mean blood flow velocity (TVmean) and tumor microvessel density (MVD) in peripheral blood supply of hepatocellular carcinoma (HCC) and the relationship between TVmean and tumor cell differentiation, to determine the noninvasive Valuate the value of HCC biological properties. Methods Twenty-five patients with HCC were enrolled in this study. The TVmean of 45 patients with normal liver tissue was detected by ultrasonography. The histopathological staining of HCC tissues was observed for the degree of cell differentiation, immunohistochemical staining for MVD, and linear correlation analysis between TVmean and MVD. Results There was a linear correlation between TVmean and MVD in the tumor of HCC. The correlation coefficient was 0.794 and the regression equation was y = 18.2764 + 1.5544x. According to TVmean≥21.3cm / s, the positive predictive value of poorly differentiated HCC was 97.4%, the negative predictive value was 87.5%; TVmean <21.3cm, the positive predictive value of differentiated HCC was 88.5% and the negative predictive value was 97.3% TVmean calculated the degree of HCC cell differentiation degree of credibility of 89.5%. Conclusion TVmean of HCC aneurysm can evaluate tumor growth status and cell malignancy before operation, and provide a useful basis for the analysis of prognosis.
