[目的]观察星状神经节阻滞(SGB)对乳腺癌患者围术期细胞免疫及体液免疫的影响。[方法]择期乳腺癌改良根治术女性患者80例,随机分成对照组和干预组,观察干预前(T1)、干预后d1(T2)和干预后d5(T3)时CD3+、CD4+、CD8+、NK细胞活性以及IgG、IgA、IgM的变化。[结果]2组各时点CD3+、CD4+、CD8+均无显著变化(P﹥0.05)。Ⅰ组T2、T3时NK活性明显低于T1(P﹤0.05或P﹤0.01),Ⅱ组仅T2时NK活性降低(P﹤0.05),T3时NK活性恢复,明显高于Ⅰ组(P﹤0.05)。与T1比较,T2、T3时2组IgG、IgA、IgM均有不同程度下降(P﹤0.05,P﹤0.01),T3时Ⅱ组IgG明显高于Ⅰ组(P﹤0.05),T2、T3时Ⅱ组IgA明显高于Ⅰ组(P﹤0.05)。[结论]术前SGB可减轻乳腺癌改良根治术患者细胞免疫和体液免疫的抑制程度。
[Objective] To observe the effect of stellate ganglion block (SGB) on cellular immunity and humoral immunity in patients with breast cancer during perioperative period. [Methods] A total of 80 female patients with modified radical mastectomy were randomly divided into control group and intervention group. The levels of CD3 +, CD4 +, CD8 +, NK before and after d1 (T2) and d5 (T3) Cell activity and changes of IgG, IgA, IgM. [Results] There were no significant changes in CD3 +, CD4 +, CD8 + at each time point in both groups (P> 0.05). The activity of NK at T2 and T3 in group Ⅰ was significantly lower than that at T1 (P <0.05 or P <0.01). The activity of NK in group Ⅱ was only decreased at T2 (P <0.05) 0.05). Compared with T1, IgG, IgA and IgM in 2 groups decreased significantly at T2 and T3 (P <0.05, P <0.01), IgG in group Ⅱ was significantly higher at T3 (P <0.05) IgA in group Ⅱ was significantly higher than that in group Ⅰ (P <0.05). [Conclusion] Preoperative SGB can reduce the degree of inhibition of cellular immunity and humoral immunity in patients with modified radical mastectomy of breast cancer.
