目的系统评价白细胞介素-12(IL-12)对结核性胸膜炎的诊断价值。方法系统检索PubMed、Embase、Web of Science、万方、中国知网、维普等数据库,检索时间为各数据库建库至2014年2月。按照设定的纳入标准筛选查找有关研究IL-12诊断结核性胸膜炎的文献。采用诊断性试验准确性质量评价工具评价纳入文献的质量。利用Meta-Disc 1.4软件进行Meta分析,计算合并灵敏度、特异度等诊断指标,并绘制综合受试者特征曲线,对诊断性试验的准确性进行综合评估。结果共纳入8篇文献,IL-12对结核性胸膜炎的合并诊断价值分别为:灵敏度为0.80[95%CI(0.76,0.84)],特异度为0.76[95%CI(0.71,0.81)],阳性似然比为3.23[95%CI(2.26,4.60)],阴性似然比为0.30[95%CI(0.20,0.45)],诊断优势比为13.57[95%CI(6.66,27.64)]。综合受试者特征曲线下面积为0.86。结论 IL-12在结核性胸膜炎的诊断中具有一定的诊断价值,可作为辅助诊断指标。
Objective To evaluate the diagnostic value of interleukin-12 (IL-12) in the diagnosis of tuberculous pleurisy. Methods The databases of PubMed, Embase, Web of Science, Wanfang, CNKI and VIP were systematically searched, and the retrieval time was from the database construction to February 2014. According to the set of inclusion criteria screening for the study of IL-12 diagnosis of tuberculous pleurisy literature. The quality of the included documents is assessed using a diagnostic testing quality assessment tool. Meta-analysis was performed with Meta-Disc 1.4 software to calculate diagnostic sensitivity and specificity, and the comprehensive characteristic curves of the subjects were drawn to evaluate the accuracy of the diagnostic tests. Results A total of 8 articles were included. The diagnostic value of IL-12 in tuberculous pleurisy were 0.80 [95% CI (0.76,0.84)], specificity of 0.76 [95% CI (0.71,0.81)], The positive likelihood ratio was 3.23 [95% CI (2.26,4.60)] and the negative likelihood ratio was 0.30 [95% CI (0.20,0.45)]. The diagnostic odds ratio was 13.57 [95% CI 6.66,27.64]. The area under the composite subject’s characteristic curve was 0.86. Conclusion IL-12 in the diagnosis of tuberculous pleurisy has a certain diagnostic value, can be used as a diagnostic indicator.
