目的探讨三维能量多普勒血流成像(3D-PDA)技术在定量评估睾丸不全扭转血流灌注量改变的应用价值。方法将20只实验狗的一侧睾丸扭转后进行3D-PDA检查,测量不同扭转度数和时间的睾丸血管指数(VI)、血流指数(FI)、血管-血流指数(VFI),并与对侧未扭转的睾丸比较;收集临床睾丸不全扭转患者8例,对双侧睾丸行3D-PDA检查。结果不同扭转度数和时间动物睾丸的VI、FI、VFI值较对照组均下降(P<0.05),且随着扭转角度和时间的增加呈逐渐降低趋势。睾丸不全扭转患者的VI、FI、VFI均明显低于其对侧正常组(P<0.01)。结论3D-PDA为判断睾丸不全扭转的血流灌注量改变提供了客观的定量参数指标。
Objective To investigate the value of three-dimensional energy Doppler flow imaging (3D-PDA) in the quantitative assessment of testicular dysfunction with changes of blood flow perfusion. Methods The testes of one side of 20 dogs were examined by 3D-PDA after torsion, and testicular vascular index (VI), blood flow index (FI) and vascular-blood flow index (VFI) were measured with different degrees of torsion and time, Contralateral non-twisted testis; collection of patients with clinical testicular torsion in 8 patients, bilateral testis line 3D-PDA examination. Results Compared with the control group, the VI, FI and VFI of testes in different degrees of torsion and time decreased significantly (P <0.05), and gradually decreased with the increase of torsion angle and time. The VI, FI and VFI in patients with testicular torsion were significantly lower than those in the contralateral normal group (P <0.01). Conclusion 3D-PDA provides an objective quantitative parameter for determining the change of perfusion of testicular torsion.
