随着新生儿重症监护病房的建立和发展,影像诊断进入新生儿领域,对新生儿缺氧缺血性脑病(HIE)的认识日益提高。由于 CT 扫描具有的优点,它已作为HIE 诊断和预后评估的重要手段,但目前国内缺乏统一确切的 CT 诊断 HIE 的客观指标。本文通过5年来我科对96例 HIE 的临床和 CT 检查,初步总结出HIE 的临床和 CT 影像特点,找出脑白质低密度改变及颅内出血尤其是蛛网膜下腔出血(SAH)的初步规律,为新生儿科医师在结合临床阅读 CT 片时的参考,并为进一步制订 CT 诊断标准提供依据。
With the establishment and development of neonatal intensive care units, imaging diagnosis enters the field of neonates, and awareness of neonatal hypoxic-ischemic encephalopathy (HIE) is increasing. Due to the advantages of CT scanning, it has been used as an important method for HIE diagnosis and prognosis evaluation. However, there is no objective index for the diagnosis of HIE in China at present. Based on the clinical and CT findings of 96 HIE patients in our department over the past five years, the clinical and CT features of HIE were initially summarized, and the preliminary rules of low density white matter changes and intracranial hemorrhage, especially subarachnoid hemorrhage (SAH), were found out , For neonatologists in the clinical reading of CT reference, and to provide a basis for the further development of diagnostic criteria for CT.
