目的探讨妊娠期糖尿病、子痫前期与代谢综合征(MS)表型的相关性。方法收集单胎妊娠健康孕妇(对照组)110例,子痫前期孕妇41例,妊娠期糖尿病(GDM)孕妇51例,GDM合并子痫前期孕妇39例,比较4组孕妇的血压、空腹血糖、空腹胰岛素、总胆固醇、甘油三脂和体重增长程度;孕期体重增长与甘油三脂、胆固醇的关系。结果子痫前期、GDM和合并组孕妇在不同孕期的甘油三酯、胆固醇、空腹血糖和空腹胰岛素均高于同期对照组;子痫前期、GDM和合并组孕妇在不同孕期及整个孕期体重增长值均高于对照组;妊娠不同时期的体重增长与同期甘油三酯、胆固醇分别呈正相关。结论 GDM和子痫前期孕妇体内均有不同程度的脂代谢紊乱及胰岛素抵抗,且与代谢综合征一样有共同的高危因素——肥胖;GDM、子痫前期孕妇与MS有高度相关性。
Objective To investigate the correlation between gestational diabetes mellitus, preeclampsia and metabolic syndrome (MS) phenotypes. Methods One hundred and ten healthy pregnant women with singleton pregnancy (control group), 41 pregnant women with preeclampsia, 51 pregnant women with gestational diabetes mellitus (GDM) and 39 pregnant women with GDM complicated with preeclampsia were enrolled in this study. Blood pressure, fasting blood glucose, Fasting insulin, total cholesterol, triglyceride and weight gain; weight gain during pregnancy and triglyceride, cholesterol. Results The levels of triglyceride, cholesterol, fasting blood glucose and fasting insulin in pregnant women in preeclampsia, GDM and combined group were significantly higher than those in the same period of pregnancy. In preeclampsia, GDM group and combined group Were higher than the control group; body weight gain at different stages of pregnancy and triglyceride, cholesterol were positively correlated. Conclusions Both pregnant women with GDM and preeclampsia have different levels of lipid metabolism disorder and insulin resistance in vivo, and have the same high risk as metabolic syndrome - obesity. GDM and preeclampsia are highly correlated with MS.
