目的:探讨双水平正压通气(BiPAP)呼吸机治疗慢性阻塞性肺病(COPD)伴II型呼吸衰竭的临床应用价值及其对循环系统变化的影响。方法:对40例慢性阻塞性肺疾病(COPD)并II型呼吸衰竭患者给予BiPAP呼吸机通气治疗,观察治疗前后PH、PaO2、PaCO2、SaO2、HR、BP等指标变化。结果:BiPAP呼吸机治疗后PaCO2、SaO2均明显升高,PaCO2明显降低,PH明显改善,循环系统变化不大,差异均有显著性(P<0.0l)。结论:双水平气道正压通气是治疗COPD并II型呼吸衰竭疗效满意,无明显并发证,值得临床推广应用。
Objective: To investigate the clinical value of bi-level positive pressure ventilation (BiPAP) ventilator in the treatment of chronic obstructive pulmonary disease (COPD) with type II respiratory failure and its effect on the circulatory system. Methods: Forty patients with chronic obstructive pulmonary disease (COPD) and type II respiratory failure were given BiPAP ventilator ventilation. The changes of PH, PaO2, PaCO2, SaO2, HR and BP before and after treatment were observed. Results: PaCO2 and SaO2 were significantly increased, PaCO2 significantly decreased, PH improved obviously, and circulatory system did not change much in BiPAP ventilator. The difference was significant (P <0.01). Conclusions: Biphasic positive airway pressure is the best choice for the treatment of COPD and type II respiratory failure. It is worth noting that it should be clinically popularized and applied.
