目的探讨CD64与CD14在急性单核细胞相关的髓系白血病免疫分型鉴别诊断中的意义,为临床诊治工作提供参考依据。方法选取2012年1月至2015年2月间收治的急性单核细胞相关的髓系白血病患者100例,采用流式细胞仪分析患者CD64、CD14抗原表达情况,对比分析检测结果。结果 CD64表达情况M0(急性粒细胞白血病微分化型)、M1(急性粒细胞白血病未成熟型)、M2(急性粒细胞白血病部分分化型)、M3(急性早幼粒细胞白血病)、M4(急性粒-单核细胞白血病)、M5(急性单核细胞白血病)阳性率分别为0.0%,0.0%,11.1%,47.4%,81.0%,100%。CD14表达情况M0、M1、M2、M3、M4、M5阳性率分别为0.0%,0.0%,0%,0%,52.4%,79.2%。45例M4/M5患者中,CD14 30例阳性,灵敏度为66.7%,特异性为100%,而CD64 41例阳性,灵敏度为93.3%,特异性为79.2%。结论 CD64诊断急性髓系白血病灵敏度显著高于CD14,而CD14诊断疾病的特异性明显高于CD64,各有优势、不足,二者相互结合应用能够明显改善检验准确度,减少误差,能够为临床诊断治疗等方面提供可靠的参考依据,值得借鉴。
Objective To investigate the significance of CD64 and CD14 in differential diagnosis of acute myeloid leukemia associated with immunophenotyping and to provide a reference for clinical diagnosis and treatment. Methods One hundred patients with acute myeloid leukemia were selected from January 2012 to February 2015. The expression of CD64 and CD14 antigen was analyzed by flow cytometry, and the results were compared and analyzed. Results The expressions of CD64 were M0 (acute myeloid leukemia differential), M1 (acute myeloid leukemia immature), M2 (acute myeloid leukemia partially differentiated), M3 (acute promyelocytic leukemia), M4 (acute The positive rate of M5 (acute monocytic leukemia) was 0.0%, 0.0%, 11.1%, 47.4%, 81.0% and 100%, respectively. The positive rates of CD14, M0, M1, M2, M3, M4 and M5 were 0.0%, 0.0%, 0%, 0%, 52.4% and 79.2% respectively. Of the 45 patients with M4 / M5, 30 were positive for CD14 with a sensitivity of 66.7% and a specificity of 100%, while 41 of CD64 were positive with a sensitivity of 93.3% and a specificity of 79.2%. Conclusions The sensitivity of CD64 in diagnosis of acute myeloid leukemia is significantly higher than that of CD14. However, the specificity of CD14 in diagnosis of disease is significantly higher than that of CD64. Each combination has advantages and disadvantages. The combination of both can significantly improve the accuracy of test, reduce the error and can be used for clinical diagnosis Treatment and other aspects provide a reliable reference, it is worth learning.
