目的观察皮肤磨削术结合光动力疗法治疗乳房外Paget病的临床疗效。方法对8例病理确诊为乳房外Paget病的患者进行肿瘤皮损的皮肤磨削术,术后外用5-氨基酮戊酸溶液封包,4h后给予氦-氖激光照射,波长635nm,能量80J/cm2,照射25min。10d后创面愈合时进行第2次光动力治疗,1周后进行第3次光动力治疗,3次为1个疗程。疗程结束后1、3和6个月观察病灶局部反应。结果疗程结束后3个月时4例病灶消失;3例病灶缩小达50%以上;1例皮损处红斑仍然较多,未获控制。结论皮肤磨削术结合光动力学疗法是乳房外Paget病的有效治疗方法。
Objective To observe the clinical efficacy of dermabrasion combined with photodynamic therapy for extramammary Paget disease. Methods Eight patients with pathologically diagnosed extramammary Paget disease were subjected to dermabrasion using a 5-aminolevulinic acid solution after external application. After 4 hours, Xenon-Krypton laser irradiation was performed. The wavelength was 635 nm and the energy was 80 J/. Cm2, irradiation for 25 minutes. The second photodynamic therapy was performed when the wound was healed after 10 days, and the third photodynamic therapy was performed after 1 week, and 3 times was a course of treatment. Local reactions were observed at 1, 3, and 6 months after treatment. Results The lesions disappeared in 3 cases 3 months after the end of treatment; the lesions were reduced by more than 50% in 3 cases; erythema in 1 lesion was still more and was not controlled. Conclusions Dermabrasion combined with photodynamic therapy is an effective treatment for extramammary Paget disease.
