目的:探讨CD44v5、CD44v6、CD44s及LN-R的表达异常和葡萄胎恶变的关系及在预测葡萄胎恶变方面的价值。方法:采用针对4种产物的单克隆抗体,S-P法免疫组织化学染色,回顾性分析了经随访证实发生恶变的葡萄胎49例(恶变组)和未发生恶变的葡萄胎72例(非恶变组)中4种产物的表达情况。结果:恶变组CD44v5的表达程度显著低于非恶变组(P<0.01);恶变组CD44v6表达程度显著高于非恶变组(P<0.01)。进一步以4种产物的表达程度为变量,进行Logistic回归判别分析,表明4种产物联合应用对葡萄胎恶变的预测敏感性率为60.5%,特异性率为84.3%,而Logistic逐步回归判别分析显示,CD44v5和CD44v6联合应用时,对葡萄胎恶变的预测敏感性率为60.5%,特异性率为88.2%。结论:CD44v5和CD44v6的表达可作为预测葡萄胎恶变的新指标。
Objective: To investigate the relationship between the expression of CD44v5, CD44v6, CD44s and LN-R and the malignancy of hydatidiform mole and the value of predicting the malignancy of hydatidiform mole. Methods: Monoclonal antibodies against four products and SP immunohistochemical staining were retrospectively analyzed in 49 cases of hydatidiform mole (malignant group) and 72 cases of non-malignant hydatidiform mole (non-malignant group) ) In four kinds of product expression. Results: The expression of CD44v5 in malignant group was significantly lower than that in non-malignant group (P <0.01). The expression of CD44v6 in malignant group was significantly higher than that in non-malignant group (P <0.01). Logistic regression analysis showed that the predictive sensitivity of the four products in combined detection of hydatidiform mole was 60.5% and the specificity was 84.3%. Logistic regression analysis showed , CD44v5 and CD44v6 combined application, the sensitivity of hydatidiform mole prediction was 60.5%, the specificity was 88.2%. Conclusion: The expressions of CD44v5 and CD44v6 can be used as new markers to predict the malignancy of hydatidiform mole.
