目的探讨早期立位促通联合核心肌群训练对痉挛型脑瘫患儿平衡及步行能力的影响。方法将符合纳入标准的80例痉挛性脑瘫患儿随机分为4组,第1组接受常规功能训练2次(30 min/次),第2组接受常规功能训练1次(30 min/次)结合立位促通训练1次(30 min/次),第3组接受常规康复训练1次(30 min/次)结合核心肌群训练1次(30 min/次),第4组接受常规功能训练1次(30 min/次)的同时进行早期立位促通1次(15 min/次)和核心肌群训练1次(15 min/次),所有项目均为每周5次,3个月为1个疗程。治疗前后4组患儿均采用Berg平衡量表和粗大运动功能测定(GMFM-88)进行评估,根据评分结果进行组内和组间比较。结果 4组患儿与组内治疗前后平衡功能和粗大运动功能参数比较(P>0.05),方差分析发现4组患儿4种不同训练方法治疗后平衡功能、粗大运动功能分值提高差异(P<0.05),两两分析发现,第3组与第4组分值的提高差异(P>0.05)。结论早期立位促通联合核心肌群训练对痉挛型脑瘫患儿平衡及步行能力有明显的促进作用,有利于改善异常步态,提高患儿生活自理能力。
Objective To investigate the effects of early standing-up of TCM, promoting core-group training on balance and walking ability of children with spastic cerebral palsy. Methods Eighty children with spastic cerebral palsy who met the inclusion criteria were randomly divided into 4 groups: group 1 received routine functional training twice (30 min / time), group 2 received routine functional training once (30 min / time) The patients in group 3 received routine rehabilitation training once (30 min / time) and core muscle group training time (30 min / time), while those in group 4 received routine function (30 min / time) Training 1 time (30 min / time) at the same time, the early standing promotion 1 (15 min / times) and core muscle training 1 (15 min / times), all items are 5 times a week, 3 Month for a course of treatment. Before and after treatment, the four groups of children were assessed by Berg Balance Scale and Gross Motor Function Test (GMFM-88), and the results were compared between groups and between groups. Results There were significant differences in balance function and gross motor function before and after treatment between 4 groups (P> 0.05). Analysis of variance showed that after 4 different training methods, the balance function and gross motor score improved <0.05). The two groups showed that the difference between the third group and the fourth group increased (P> 0.05). Conclusions The combination of early standing and promoting with core muscle training can obviously improve the balance and walking ability of children with spastic cerebral palsy, which is helpful to improve the abnormal gait and improve their self-care ability.
