目的探讨过氟化碳液体(perfluorocarbonliquid,PFCL)在晶体及玻璃体视网膜手术(vitre-oretinalsurgery,VR术)中的应用价值。方法应用PFCL(法国产过氟萘烷,perfluorodeclin,纯度100%,C10F18)治疗2例(2只眼)晶体脱位于玻璃体内,采用玻璃体切除,PFCL漂浮晶体于前房,再行常规的白内障摘除术。在VR术中应用PFCL治疗37例(37只眼)复杂性视网膜脱离(网脱)、2例(2只眼)外伤性网脱和1例增殖性糖尿病视网膜病变(PDR),采用玻璃体切除、膜剥离、PFCL使用、气液交换、眼内激光、惰性气体(SF6、C3F8)及硅油眼内填充。结果晶体脱位2例,顺利取出,术后视力均较术前明显提高。VR术40例,使用PFCL一次,近期(出院时)观察有效35例(35只眼,87.5%),失败5例(5只眼,12.5%)。视力与术前比较提高者24眼(64.9%),不变9眼(24.3%),降低4眼(17.14%)。27例随访时间为3~11月(平均6.5月),有效25只眼(92.6%),视力≥0.05,占48%。结论PFCL可安全有效的治疗晶体脱位于玻璃体内,提高了VR术?
Objective To investigate the value of perfluorocarbon liquid (PFCL) in vitre-oretinalsurgery (VR). Methods Two cases (2 eyes) with PFCL (perfluorodeclin, purity 100%, C10F18) were treated with vitrectomy and vitrectomy. PFCL floating crystals were placed in the anterior chamber and followed by conventional cataract extraction Surgery. 37 cases (37 eyes) complicated with retinal detachment (net off), 2 cases (2 eyes) traumatic reticular detachment and 1 case of proliferative diabetic retinopathy (PDR) were treated with PFCL in VR. Vitrectomy, Membrane stripping, PFCL use, gas-liquid exchange, intraocular laser, inert gas (SF6, C3F8) and silicone oil intraocular filling. Results 2 cases of dislocation of the crystal were successfully removed and the postoperative visual acuity was significantly higher than that before operation. Forty patients underwent VR, one with PFCL, and 35 (35 eyes, 87.5%) in the short term (at discharge). Five patients (5 eyes, 12.5%) failed. There were 24 eyes (64.9%) with visual acuity and preoperative improvement, 9 eyes (24.3%) unchanged and 4 eyes (17.14%) with unchanged vision. Twenty-seven patients were followed up for 3-11 months (mean 6.5 months), effective 25 eyes (92.6%), visual acuity ≥ 0.05, accounting for 48%. Conclusion PFCL can be used safely and effectively to treat the dislocation of the lens in the vitreous and improve the VR technique.
