目的观察原发性甲状腺功能减退症(甲减)患者激素替代治疗前后心肌酶、血脂的变化。方法 56例甲状腺功能减退症患者采用口服左旋甲状腺激素替代治疗,观察治疗后4、12周患者心肌酶及血脂的变化。结果与治疗前比患者治疗后肌酸激酶(CK)、同工酶(CKMB)、天门冬氨酸氨基转移酶(AST)、乳酸脱氢酶(LDH)水平明显下降(P<0.05或P<0.01)。与治疗前比患者治疗后血清总胆固醇(TC)、甘油三酯(TG)、和低密度脂蛋白(LDL)水平明显下降(P<0.05或P<0.01),而高密度脂蛋白(HDL)水平比较差异无统计学意义(P>0.05)。结论左旋甲状腺激素替代治疗可以降低甲状腺功能减退症患者心肌酶和改善血脂紊乱,能很大程度上减少甲状腺功能减退症合并心脑血管疾病的发生。
Objective To observe the changes of myocardial enzymes and lipids in patients with primary hypothyroidism (hypothyroidism) before and after hormone replacement therapy. Methods Fifty-six patients with hypothyroidism underwent oral L-thyroid hormone replacement therapy. Changes of myocardial enzymes and blood lipids were observed at 4 and 12 weeks after treatment. Results Compared with those before treatment, the levels of creatine kinase (CK), aspartate aminotransferase (AST) and lactate dehydrogenase (LDH) in patients after treatment were significantly lower than those before treatment (P <0.05 or P < 0.01). Compared with pretreatment patients, serum total cholesterol (TC), triglyceride (TG), and low density lipoprotein (LDL) levels were significantly decreased (P <0.05 or P <0.01), while high density lipoprotein (HDL) The difference was not statistically significant (P> 0.05). Conclusion L-thyroid hormone replacement therapy can reduce myocardial enzymes in patients with hypothyroidism and improve dyslipidemia, can greatly reduce the incidence of hypothyroidism with cardiovascular and cerebrovascular diseases.
