目的研究口服葡萄糖耐量试验(Oral Glucose Tolerance Test,OGTT)正常健康人空腹、2 h血糖、胰岛素水平、胰岛素抵抗指数(Homeostasis Model Assessment-Insulin Resistance,HOMA-IR)与心血管危险因素之间的相关性。方法北京市社区人群心血管危险因素调查研究、OGTT正常健康人506例,男性356例,女性150例,年龄17~75岁,年龄中位数33岁。全自动生化分析仪检测血糖、血脂、尿酸等血液生化指标。将研究对象按照空腹、OGTT 2 h血糖、胰岛素水平及HOMA-IR四分位数分组,采用方差和协方差分析进行组间心血管危险因素指标的比较。结果校正年龄、性别、体质量指数、吸烟和饮酒之后的协方差分析结果显示,与空腹血糖相关联的因素是舒张压(F=3.86,P<0.05)和心率(F=6.90,P<0.05)。与2 h血糖相关联的因素只有心率(F=4.09,P<0.05)。与空腹胰岛素相关联的因素是舒张压(F=4.14,P<0.05)、心率(F=3.79,P<0.05)和血浆甘油三酯(F=6.34,P<0.05)。与2 h胰岛素相关联的因素是腰臀比(F=6.905,P<0.05)、心率(F=4.61,P<0.05)、血浆甘油三酯(F=5.92,P<0.05)、高密度脂蛋白胆固醇(F=3.43,P<0.05)。与HOMA-IR相关联的因素是腰臀比(F=2.928,P<0.05)、心率(F=3.811,P<0.05)、血浆甘油三酯(F=9.262,P<0.05)、高密度脂蛋白胆固醇(F=2.738,P<0.05)和尿酸(F=3.390,P<0.05)。结论在糖耐量试验正常健康人中,与空腹、2 h血糖比较,空腹、2 h胰岛素以及HOMA-IR水平与更多的心血管危险因素相关联。
Objective To investigate the relationship between fasting, 2-hour blood glucose, insulin level, Homeostasis Model Assessment-Insulin Resistance (HOMA-IR) and cardiovascular risk factors in oral Oral Glucose Tolerance Test (OGTT) Sex. Methods A total of 506 OGTT normal healthy subjects were enrolled in this study. They included 356 males and 150 females, aged 17-75 years with a median age of 33 years. Automatic biochemical analyzer for blood glucose, blood lipids, uric acid and other blood biochemical indicators. The subjects were grouped according to fasting, 2-hour OGTT glucose, insulin levels and HOMA-IR quartiles, and variance and covariance analysis were used to compare cardiovascular risk factors between groups. Results The results of covariance analysis after adjustment for age, sex, body mass index and smoking and drinking showed that the factors associated with fasting blood glucose were diastolic pressure (F = 3.86, P <0.05) and heart rate (F = 6.90, P <0.05 ). The only factors associated with 2-h blood glucose were heart rate (F = 4.09, P <0.05). The factors associated with fasting insulin were diastolic blood pressure (F = 4.14, P <0.05), heart rate (F = 3.79, P <0.05) and plasma triglyceride (F = 6.34, P <0.05). The factors associated with 2 h of insulin were waist-hip ratio (F = 6.905, P <0.05), heart rate (F = 4.61, P <0.05), plasma triglyceride Protein cholesterol (F = 3.43, P <0.05). The factors associated with HOMA-IR were waist-hip ratio (F = 2.928, P <0.05), heart rate (F = 3.811, P <0.05), plasma triglycerides (F = 9.262, Protein cholesterol (F = 2.738, P <0.05) and uric acid (F = 3.390, P <0.05). Conclusions In fasting, 2-h blood glucose, fasting, 2-h insulin and HOMA-IR levels are associated with more cardiovascular risk factors in normal glucose tolerance test subjects.
