目的:比较动脉有创直接测压(IABP)与无创上臂袖带测压(NIBP)差异。方法:随机选择择期手术患者450例,局麻下行桡动脉或足背动脉穿刺并置管直接测压,上臂无创袖带测压,比较不同血压范围IABP与NIBP差异。结果 :IABP与左、右上臂NIBP收缩压压差:左桡动脉组分别为(18.20±0.67、15.27±0.62)mm Hg,右桡动脉组分别为(18.54±0.79、14.68±0.77)mm Hg,左足背动脉组分别为(41.98±2.26、40.54±1.77)mm Hg,右足背动脉组分别为(37.57±0.01、37.93±2.90)mm Hg。左、右上臂NIBP收缩压压差(2.84±0.45)mm Hg,舒张压压差(3.19±0.34)mm Hg,平均压压差(2.85±0.35)mm Hg,右侧>左侧。结论:成人清醒状态下动脉直接测压高于袖带测压,收缩压高出约20 mm Hg左右,并证实30岁以后收缩压随年龄增长而逐渐升高,舒张压增高至50岁后逐渐下降。
PURPOSE: To compare the differences between arterial invasive direct manometry (IABP) and non-invasive upper arm cuff manometry (NIBP). Methods: A total of 450 patients undergoing elective surgery were randomly selected. Local anesthesia was performed with radial artery or dorsalis pedis artery puncture and direct catheterization. Noninvasive cuff pressure measurement was performed on the upper arm. The differences of IABP and NIBP were compared among different blood pressure ranges. Results: The systolic pressure difference between IABP and left and right upper arm was (18.20 ± 0.67,15.27 ± 0.62) mm Hg in the left radial artery group and (18.54 ± 0.79 and 14.68 ± 0.77) mm Hg in the right radial artery group, The left and right dorsal arteries were (41.98 ± 2.26 and 40.54 ± 1.77) mm Hg, and the right and left dorsal arteries were (37.57 ± 0.01, 37.93 ± 2.90) mm Hg respectively. Left and right upper arm NIBP systolic pressure difference (2.84 ± 0.45) mm Hg, diastolic blood pressure difference (3.19 ± 0.34) mm Hg, mean pressure difference (2.85 ± 0.35) mm Hg, right> left. CONCLUSIONS: In adult conscious patients, the direct arterial pressure is higher than cuff pressure and the systolic blood pressure is about 20 mm Hg. It is also demonstrated that after 30 years of age systolic blood pressure gradually increases with age and diastolic blood pressure increases gradually after 50 years of age decline.
