目的探讨基因芯片技术在检测结核分枝杆菌及其利福平耐药性中的应用效果。方法分别收集我院临床诊断肺结核病患者的痰液标本共86例,疑似肺结核病患者的痰液标本30例,临床诊断肺结核病患者的痰液标本1例4份,疑似肺结核病患者标本1例3份,分别用于Gene Xpert MTB/RIF、痰涂片、传统罗氏痰培养及药敏试验,对比Gene Xpert MTB/RIF检测与其它检测结果之间的差异。当双方差异显著时,认为结核分枝杆菌阳性率高的一方,检测方法更优,当双方无显著差异时,认为比较双方检测效果等同。结果 86例临床确诊肺结核病例标本中,Gene Xpert MTB/RIF检测结果与其他检测结果(痰涂片、痰培养、药敏试验)比较,无显著差异,即P>0.05;30例疑似肺结核病例标本中,Gene Xpert MTB/RIF检测结果与其他检测结果(痰涂片、痰培养)比较,P<0.05,其中Gene Xpert MTB/RIF检出结核分枝杆菌阳性率为93.3%,痰涂片为60%,痰培养为63.3%。结论基因芯片技术(Gene Xpert MTB/RIF)检测结核分枝杆菌及利福平的耐药性快速准确,其检测的特异性和敏感性优于痰涂片和痰培养,与药敏试验之间无差异,可以应用于结核分枝杆菌耐药性的检测。
Objective To investigate the application of gene chip technology in the detection of Mycobacterium tuberculosis and its rifampicin resistance. Methods Totally 86 cases of sputum samples were collected from our hospital for clinical diagnosis of pulmonary tuberculosis. Thirty sputum samples from suspected pulmonary tuberculosis patients, 4 from sputum samples from 4 patients with suspected pulmonary tuberculosis, 1 from suspected pulmonary tuberculosis patients 3 copies respectively for Gene Xpert MTB / RIF, sputum smear, traditional Roche sputum culture and drug sensitivity test to compare the difference between Gene Xpert MTB / RIF test and other test results. When the difference between the two was significant, it was considered that the method with higher positive rate of Mycobacterium tuberculosis was better, and when there was no significant difference between the two, it was considered that the test results were equal. Results Among the 86 clinically diagnosed cases of pulmonary tuberculosis, there was no significant difference between Gene Xpert MTB / RIF test results and other test results (sputum smear, sputum culture, drug susceptibility test), ie, P> 0.05. Thirty cases of suspected pulmonary tuberculosis (P <0.05). The positive rate of Mycobacterium tuberculosis detected by Gene Xpert MTB / RIF was 93.3% and the sputum smear was 60 %, Sputum culture was 63.3%.
