目的 :研究慢性呼吸衰竭患者的血小板功能及抗凝、纤溶因子的变化。方法 :将慢性呼吸衰竭患者分为发作期 (A组 ,32例 ) ,缓解期 (B组 ,30例 )和正常对照 (C组 ,2 6例 )。分别检测各组的血小板粘附率(PAdT) ,聚集率 (PAdT) ,血栓烷B2 (TXB2 ) ,β 血小板球蛋白 (β TG) ,血管性假血友病因子 (vWF) ,6 keto 前列腺素 (6 keto PGF1α) ,抗凝血酶Ⅲ活性 (AT Ⅲ :C) ,纤维蛋白原 (Fg) ,D -二聚体 (DD)。结果 :慢性呼吸衰竭发生时PAdT ,TXB2 ,β TG ,vWF ,6 keto PGF1α,Fg ,DD明显高于正常对照 ,PAgT ,AT Ⅲ :C低于正常 ,差异均有显著意义。缓解时PAgT继续增高 ,与正常组比较差异有非常显著意义。PAgT由低于正常 ,转为高于正常。vWF ,TXB2 ,6 keto PGF1α,Fg ,DD虽较发病时有明显下降 ,但仍高于正常。β TG ,AT Ⅲ :C逐渐降低 ,与正常比较 ,差异无显著意义。结论 :慢性呼吸衰竭发生过程中 ,血小板被激活 ,其粘附释放功能增强 ,机体处于高凝状态 ,并有轻度的继发性纤溶发生
Objective: To study the changes of platelet function, anticoagulation and fibrinolysis in patients with chronic respiratory failure. Methods: Patients with chronic respiratory failure were divided into episode (group A, 32 cases), remission (group B, 30 cases) and normal control group (group C, 26 cases). The levels of PAdT, PAdT, TXB2, βTG, vWF, 6 keto prostaglandin (6 keto PGF1α), antithrombin Ⅲ activity (AT Ⅲ: C), fibrinogen (Fg) and D - dimer (DD). Results: The levels of PAdT, TXB2, β TG, vWF, 6 keto PGF1α, Fg and DD in chronic respiratory failure group were significantly higher than those in normal control group. The levels of PAgT and AT Ⅲ: C were significantly lower than those in normal group. PAgT continued to increase in relief, compared with the normal group has a very significant difference. PAgT from below normal, into higher than normal. vWF, TXB2, 6 keto PGF1α, Fg, DD although significantly decreased compared with the onset, but still higher than normal. β TG, AT Ⅲ: C gradually decreased, compared with the normal, no significant difference. Conclusion: In the process of chronic respiratory failure, platelets are activated, the adhesion and release functions are enhanced, the body is hypercoagulable, and mild secondary fibrinolysis occurs
