背景:血小板参数变化可以反映人体凝血功能,但是关于血小板参数在血栓性疾病患者中变化的研究,没有一致的说法,还存在争论。目的:观察脑梗死患者血小板参数变化的规律,探讨使用血细胞分析仪观察患者血小板参数变化是否可行。设计:以患者和健康人对研究对象,病例-对照的验证性研究。单位:一所大学的检验科和内科。对象:2002-01/03华中科技大学同济医学院附属协和医院神经内科确诊的初次发病的脑梗死患者37例,其中男28例,女9例;年龄45~87岁。对照组为37例健康体检者,其中男20例,女17例;年龄40~60岁。方法:用血细胞分析仪检测37例脑梗死患者的血小板分布宽度、血小板平均容积、大血小板比率、血小板数量的变化,同时以健康体检者作对照。主要观察指标:两组对象的血小板各参数变化。结果:血小板分布宽度与大血小板比率、大血小板比率与血小板平均容积之间为正相关关系(r=0.99,0.92,P<0.001),血小板数量与血小板平均容积、大血小板比率之间为负相关(r=-0.58,-0.59,P<0.05)。脑梗死患者的血小板分布宽度、血小板平均容积、大血小板比率犤(14.51±2.88)%,(10.95±1.48)fl,(34.24±11.23)%犦均高于健康体检者犤(12.86±2.02)%,(10.19±1.29)fl,(28.47±9.41)%犦,差异有显著性意义(P分别为0.01931,0.02177,0.026
BACKGROUND: Changes in platelet parameters reflect the coagulation function of the human body. However, there is no consensus about the changes in platelet parameters in patients with thrombotic diseases. There is still controversy. Objective: To observe the changes of platelet parameters in patients with cerebral infarction and to explore the feasibility of using hematology analyzer to observe the changes of platelet parameters in patients. Design: A confirmatory study of patients and healthy subjects versus controls, case-controls. Unit: a university laboratory and internal medicine. PARTICIPANTS: Thirty-seven patients with first-onset cerebral infarction diagnosed in the Department of Neurology, Union Hospital, Tongji Medical College, Huazhong University of Science and Technology from January to March 2002 included 28 males and 9 females, aged 45-87 years. The control group of 37 healthy people, including 20 males and 17 females; aged 40 to 60 years. Methods: The blood platelet analyzer was used to detect the distribution of platelet, the average platelet volume, the ratio of large platelet and the number of platelets in 37 patients with cerebral infarction. MAIN OUTCOME MEASURES: Changes in platelet parameters of two groups of subjects. Results: There was a positive correlation between platelet distribution width and large platelet ratio, large platelet ratio and average platelet volume (r = 0.99, 0.92, P <0.001). There was a negative correlation between platelet count, average platelet volume and large platelet ratio (r = -0.58, -0.59, P <0.05). The width of platelet distribution, average platelet volume and large platelet ratio in patients with cerebral infarction were (14.51 ± 2.88)%, (10.95 ± 1.48) and (34.24 ± 11.23)%, respectively, higher than those in healthy subjects (12.86 ± 2.02% , (10.19 ± 1.29) fl, (28.47 ± 9.41)% 犦, the difference was significant (P = 0.01931,0.02177,0.026
