Boys with acute lymphoblastic leukemia (ALL) have a poorer prognosis than girls, and this may be a “refuge” for the testicles to become residual cells. Some people advocate the prevention of early preventive radiation on both sides of the testes, and evaluated in clinical trials. The author of this radiation therapy recommendation argues that infertility is mainly caused by systemic chemotherapy and has nothing to do with testicular endocrine function because the latter are less damaged during irradiation. Although the reproductive function of the testis can indeed be harmed by anti-ALL chemotherapy and rarely resume promptly, one adult male with ALL has been reported to have normal childbirth shortly after completion of a long course of continuous treatment.
