目的比较小口径气管导管间歇正压通气 (IPPV)和高频喷射通气 (HFJV)在支撑喉镜术中的通气效果。方法择期支撑喉镜下声带息肉摘除术病人 30例 ,ASAⅠ~Ⅱ级 ,随机分为IPPV组(Ⅰ组 )和HFJV组 (H组 )各 15例 ,两组均插入内径 5 5mm或 6 0mm气管导管 ,分别于麻醉前 ,通气 5、10、15、2 0分钟采集动脉血作血气分析。结果H组术中PaCO2 明显升高 (P <0 0 1)并产生高碳酸血症 ;I组术中PaCO2 无明显变化 (P >0 0 5 ) ,两组术中PaO2 均明显升高 (P <0 0 1)。结论HFJV和IPPV都能满足对机体的供氧 ,但HFJV易导致二氧化碳蓄积 ,IPPV则能保证呼吸 ,安全可靠。
Objective To compare the ventilatory effects of small-caliber IPPV and high-frequency jet ventilation (HFJV) in supportive laryngoscopy. Methods Thirty ASA Ⅰ ~ Ⅱ patients with vocal cord polyp removal under elective laryngoscope were randomly divided into IPPV group (group Ⅰ) and HFJV group (group H). Fifteen patients were divided into two groups, 15 mm in diameter and 60 mm in diameter Catheters, respectively, before anesthesia, ventilation 5,10,15,2 0 minutes arterial blood collected for blood gas analysis. Results The intraoperative PaCO2 was significantly increased (P <0.01) and hypercapnia was found in group H. The PaCO2 in group I was not significantly changed (P> 0.05), and PaO2 was significantly increased in both groups (P <0 0 1). Conclusion Both HFJV and IPPV can supply oxygen to the body, but HFJV can easily lead to carbon dioxide accumulation while IPPV can ensure the respiration, which is safe and reliable.
