目的:观察脊髓慢性受压不同时间以及手术减压后不同时间受压节段神经细胞凋亡的变化。方法:将56只雄性山羊随机分为对照组(8只)、颈脊髓受压3个月组(24只)和6个月组(24只),同时将受压迫3、6个月组再分为减压术前及术后1、3个月组(每组8只)。实验动物通过颈椎前路钛合金螺钉和后路钢板置入产生慢性压迫。各组在相应时间点处死动物后取压迫节段脊髓标本,应用TUNEL技术和免疫组织化学方法检测受压脊髓不同部位神经细胞的凋亡情况。结果:脊髓慢性受压3、6个月后在受压节段不同区域均可见凋亡细胞,受压6个月组较受压3个月组有更多的凋亡细胞数(P<0.05)。后角胶质细胞凋亡变化幅度最大,而后角神经元凋亡变化幅度最小。减压手术后1个月脊髓各区凋亡细胞均减少,减压后3个月时减少更为明显(P<0.05)。结论:颈脊髓慢性受压后不同时间在脊髓不同区域出现不同数量的凋亡细胞,压迫时间越长凋亡细胞数越多;减压手术后脊髓受压区域凋亡细胞数目开始减少,术后时间越长凋亡细胞数目越少。
OBJECTIVE: To observe the changes of neural cell apoptosis in compression section at different time after spinal cord compression and at different time after decompression. Methods: Fifty-six male goats were randomly divided into control group (n = 8), cervical spinal cord compression group (n = 24) and 6 months group Divided into decompression preoperative and postoperative 1, 3 months group (8 in each group). Experimental animals through the cervical anterior titanium screws and posterior plate into the chronic compression. The rats in each group were sacrificed at corresponding time points to take the spinal cord of the compression segment. TUNEL and immunohistochemistry were used to detect the apoptosis of nerve cells in different parts of the compressed spinal cord. Results: Apoptotic cells were observed in different regions of the compression segment at 3 and 6 months after chronic compression of the spinal cord. Compared with the 3-month compression group, the number of apoptotic cells was significantly increased in the 6-month compression group (P <0.05 ). Apoptosis changes in the posterior horn of the largest, while the smallest changes in the angle of neurons apoptosis. The number of apoptotic cells in spinal cord decreased at 1 month after decompression and decreased more significantly at 3 months after decompression (P <0.05). CONCLUSIONS: After chronic compression of cervical spinal cord, different numbers of apoptotic cells appeared in different regions of spinal cord at different times, and the more the compression time, the more apoptosis cells. The number of apoptotic cells in spinal cord compression area began to decrease after decompression surgery. The longer the number of apoptotic cells, the less.
