[目的]通过检测内皮钙黏附蛋白(E-CD)和CD44蛋白的表达水平,检测门静脉癌栓中DNA倍体的比例,探讨其在肝癌肝内转移中的作用。[方法]采用免疫组化方法检测33例门静脉癌栓中E-CD和CD44蛋白的表达,在光镜下对33例门静脉癌栓石蜡标本中肝癌细胞染色体倍数按照超5倍体、2倍体和异倍体进行统计。并与各自原发灶和癌旁组织进行对照。[结果]①E-CD蛋白的表达:门静脉癌栓、肝癌原发灶、癌旁组织的平均吸光度分别为0.015 2±0.003 6,0.052 7±0.007 1和0.100 8±0.006 3。②CD44蛋白的表达:门静脉癌栓、肝癌原发灶、癌旁组织的平均吸光度分别为0.015 9±0.002 4,0.053 1±0.009 2和0.107 4±0.002 9。③肝癌细胞多倍体检测:肝癌原发灶组超5倍体比例为(25±6)%,而门静脉癌栓组是(58±9)%。各组间和各两组间差异均有统计学意义(均P<0.01)。[结论]门静脉癌栓中E-CD和CD44蛋白表达减少以及DNA超5倍体的增多是肝癌细胞向门静脉转移和门静脉癌栓向肝内转移的重要原因。
[Objective] To detect the ratio of DNA ploidy in portal vein tumor thrombus by detecting the expression of E-cadherin (E-CD) and CD44 protein and to explore its role in intrahepatic metastasis of liver cancer. [Methods] The expression of E-CD and CD44 in 33 cases of portal vein tumor thrombus was detected by immunohistochemistry. The chromosome number of hepatic cancer cells in 33 cases of portal vein tumor thrombi and paraffin embedded in paraffin-embedded specimens were detected by light microscope. And aneuploidy statistics. And with their respective primary tumor and adjacent tissues control. [Results] ① The expression of E-CD protein: The average absorbance of portal vein tumor thrombus, primary tumor and adjacent tissue of liver cancer were 0.015 2 ± 0.003 6,0.052 7 ± 0.007 1 and 0.100 8 ± 0.006 3, respectively. ② The expression of CD44 protein: The average absorbance of portal vein tumor thrombus, primary tumor and adjacent tissue of liver cancer were 0.015 9 ± 0.002 4,0.053 1 ± 0.009 2 and 0.107 4 ± 0.002 9, respectively. ③ The polyploidy detection of hepatocellular carcinoma cells: The proportion of ultra-five-fold in primary hepatocellular carcinoma was (25 ± 6)%, while it was (58 ± 9)% in portal vein tumor embolus group. There were significant differences among the groups and between the two groups (all P <0.01). [Conclusion] The decrease of E-CD and CD44 protein expression in portal vein tumor thrombus and the increase of DNA super-five-fold are the important reasons for the metastasis of hepatocellular carcinoma to the portal vein and the portal vein tumor thrombus to the liver.
