目的探索白细胞介素28B(interleukin-28B,IL-28B)基因rs12979860位点与乙型肝炎病毒(hepatitis B virus,HBV)感染后自发清除及干扰素治疗应答的关系。方法检索2010年1月1日~2015年8月31日Pub Med、Embase、中文科技期刊全文数据库、万方数据库、中国生物医学文献数据库、中国期刊全文数据库公开发表文献,对符合纳入和排除标准文献提取数据后采用Stata 12.0软件进行Meta分析。结果共纳入12篇文献,根据研究目的分为两组:A组为HBV自发清除组和持续感染组,共5篇文献;B组:干扰素治疗应答组和非应答组,共9篇文献。Meta分析结果:rs12979860位点与HBV感染后自发清除无关联(OR=0.96,95%CI:0.76~1.20,P=0.700);rs12979860位点与乙型肝炎e抗原阳性慢性HBV感染后干扰素治疗应答无关联(OR=0.93,95%CI:0.43~2.00,P=0.848),而突变基因型TC+TT降低了乙型肝炎e抗原阴性慢性HBV感染后干扰素治疗应答(OR=0.47,95%CI:0.25~0.87,P=0.016)。结论 IL-28B基因的rs12979860位点突变基因型TC+TT降低了乙型肝炎e抗原阴性慢性HBV感染后干扰素治疗的应答。
Objective To explore the relationship between rs12979860 locus of interleukin-28B (IL-28B) gene and spontaneous clearance after hepatitis B virus (HBV) infection and response to interferon treatment. METHODS: The published articles published from January 1, 2010 to August 31, 2015, Pub Med, Embase, Chinese Science and Technology Journals Full-text Database, Wanfang Database, China Biomedical Literature Database, and Chinese Journal Full-text Database, Literature data were extracted using Stata 12.0 software Meta analysis. Results A total of 12 articles were included and divided into two groups according to the purpose of the study: A group of spontaneous HBV clearance group and persistent infection group, a total of 5 articles; B group: interferon treatment response group and non-response group, a total of 9 articles. Meta analysis showed no association between rs12979860 locus and spontaneous clearance after HBV infection (OR = 0.96, 95% CI: 0.76-1.20, P = 0.700); rs12979860 locus was associated with hepatitis B e antigen positive chronic HBV infection after interferon treatment (OR = 0.93, 95% CI: 0.43-2.00, P = 0.848), while the mutant genotype TC + TT reduced the interferon response to hepatitis B e antigen negative chronic HBV infection (OR = 0.47, 95 % CI: 0.25-0.87, P = 0.016). Conclusion The genotype TC + TT of rs12979860 locus of IL-28B gene reduces the response to interferon therapy after hepatitis B e antigen negative chronic HBV infection.
