目的:评价复方丹参注射液辅助治疗小儿支原体肺炎(mycoplasma pneumoniae pneumonia,MPP)的临床疗效。方法:检索数据库:美国国立医学图书馆(PubMed,1966-2014年)、Cochrane图书馆、中国生物医学文献数据库(CBM)、中文期刊全文数据库(CNKI,1979-2014年)、中国科技期刊数据库(VIP,1989-2014年)、万方学位及会议论文数据库(1985-2014年),所有检索时间截止到2014年7月。收集所有关于复方丹参注射液辅助治疗小儿支原体肺炎的随机对照试验,由两名评价员独立进行文献的筛选,对纳入的研究进行质量评价和资料提取,用RevMan5.3软件进行Meta分析。结果:本次研究共纳入12篇RCT,1 117个病例。Meta分析结果显示复方丹参注射液辅助治疗小儿支原体肺炎的总有效率[RR=0.13,95%CI(0.09,0.16)],合并效应检验Z=6.95,P<0.00001;退热时间[WMD=-0.24,95%CI(-0.44,-0.05)],合并效应检验Z=2.47,P=0.01;咳嗽消失时间[WMD=-2.41,95%CI(-3.27,-1.54)],合并效应检验Z=5.44,P<0.00001;啰音消失时间[WMD=-2.75,95%CI(-3.60,-1.90)],合并效应检验Z=6.36,P<0.00001;X线消失时间[WMD=-2.58,95%CI(-3.64,-1.51)],合并效应检验Z=4.74,P<0.001;结论:复方丹参注射液辅助治疗小儿支原体肺炎在总有效率、退热时间、咳嗽好转时间、肺部啰音消失时间、X线阴影消失时间等方面疗效均优于常规治疗。由于本次纳入研究的RCT多数质量较低,现有的证据还不足以得出十分肯定的结论,今后需要设计更多的严谨、高质量、大样本、多中心的随机对照试验进行验证。
Objective: To evaluate the clinical efficacy of Fufang Danshen injection in the treatment of mycoplasma pneumoniae pneumonia (MPP). METHODS: The databases were searched: PubMed (1966-2014), Cochrane Library, China Biomedical Literature Database (CBM), Chinese Journal Full-text Database (CNKI, 1979-2014), Chinese Science and Technology Periodical Database VIP, 1989-2014), Wanfang Degrees and Conference Papers Database (1985-2014), all of which are up to July 2014. All randomized controlled trials on the adjuvant treatment of Mycoplasma pneumoniae with compound Salvia Miltiorrhiza bunge injection were collected. Two reviewers independently screened the literature, evaluated the quality of the included studies, and extracted the data. Meta-analysis was performed using RevMan5.3 software. Results: Twelve RCTs and 1 117 cases were included in this study. The results of Meta analysis showed that the total effective rate of compound Danshen injection adjuvant treatment of children with mycoplasmal pneumonia [RR = 0.13,95% CI (0.09,0.16)], the combined effect test Z = 6.95, P <0.00001; 0.24, 95% CI (-0.44, -0.05)], combined effect test Z = 2.47, P = 0.01; cough disappear time [WMD = -2.41,95% CI (-3.27, -1.54) = 5.44, P <0.00001; The rales disappear time [WMD = -2.75,95% CI (-3.60, -1.90)], the combined effect test Z = 6.36, P <0.00001; 95% CI (-3.64, -1.51)], the combined effect test Z = 4.74, P <0.001; Conclusion: The compound Danshen injection adjuvant treatment of children with mycoplasma pneumonia in the total effective rate of antipyretic time, cough improved time, Tone disappear time, X-ray shadow disappear time and other aspects are better than conventional treatment. As most of the RCTs included in this study are of low quality, the available evidence is not enough to reach a very positive conclusion. More rigorous, high-quality, large-sample, multicenter randomized controlled trials will need to be designed in the future.
