目的:探讨常规脑电图(electroencephalogram,EEG)、视频脑电图(video electroencephalogram,VEEG)、单光子发射计算机断层扫描(single photon emission computed tomography,SPECT)、头颅CT(computerized tomography)及MRI(magnetic resonance imaging)检查在癫痫诊断中的价值。方法:对126例已确诊的癫痫病人分别进行EEG、VEEG、SPECT、CT及MRI检查,对EEG、VEEG、SPECT、CT、MRI定位致痫灶的情况进行对比研究。结果:EEG异常126例,局灶棘波52例(41.3%);VEEG异常126例,局灶棘波114例(90.5%),两组局灶棘波检出率有显著性差异(P<0.01)。SPECT异常105例(83.3%),MRI异常32例(占55例的58.2%),CT异常5例(占48例的10.4%)。结论:VEEG、EEG、SPECT、MRI检查结合临床表现,对难治性癫痫病人术前致痫灶的定位和指导手术治疗有较大的应用价值。
Objective: To investigate the value of conventional electroencephalogram (EEG), video electroencephalogram (VEEG), single photon emission computed tomography (SPECT), computerized tomography and MRI resonance imaging in the diagnosis of epilepsy. Methods: EEG, VEEG, SPECT, CT and MRI were performed on 126 confirmed cases of epilepsy. Contrastive study was made on the epileptic foci of EEG, VEEG, SPECT, CT and MRI. Results: There were 126 cases of EEG abnormality, 52 cases of focal spikes (41.3%), 126 cases of VEEG abnormalities and 114 cases of focal spikes (90.5%). The detection rates of focal spikes were significantly different between the two groups (P < 0.01). There were 105 cases (83.3%) of SPECT abnormalities, 32 cases of MRI abnormalities (58.2% of 55 cases), and 5 cases of CT abnormalities (10.4% of 48 cases). Conclusion: VEEG, EEG, SPECT, MRI combined with clinical manifestations, the treatment of intractable epilepsy patients with epileptic foci positioning and guiding the surgical treatment of great value.
