目的观察甲苯胺红不加热血清试验(TRUST)、酶联免疫吸附试验(ELISA)与胶体金法联合筛查梅毒的效果。方法对243例不同期梅毒患者以TRUST、ELISA与胶体金法进行联合筛查检测,以3种试验中两种及以上试验阳性判别为联合筛查试验阳性,同时全部标本以明胶颗粒凝集试验(TPPA)检测作为对照,分别计算4种方法检测各期梅毒的阳性率;对联合筛查试验和对照试验各期梅毒检测结果进行比较。结果 TRUST潜伏期、三期梅毒阳性率低于其他3种方法,一期和二期梅毒阳性率与其他3种方法比较无显著性差异(P均>0.05),治愈后复查者阴转率则明显高于其他3种方法(P均<0.01);ELISA潜伏期梅毒阳性率低于胶体金法和TPPA,但差异均无统计学意义(P均>0.05),其余各期阳性率与二者相同;胶体金法潜伏期和一期梅毒阳性率略低于TPPA,三期梅毒阳性率低于ELISA和TPPA,但差异均无统计学意义(P均>0.05);各期梅毒联合筛查阳性率与TPPA法对照组比较,差异均无统计学意义(P均>0.05)。结论用TRUST、ELISA与胶体金法联合筛查各期梅毒均可取得良好的检出效果,结合病史和临床症状,还可以监测病程和疗效。
Objective To observe the effect of TRUST, ELISA and colloidal gold in the screening of syphilis. Methods Totally 243 cases of syphilis with different stages were screened by TRUST, ELISA and colloidal gold method. Two or more of the three tests were positive for combined screening test, and all samples were detected by gelatin particle agglutination test TPPA) as a control, were calculated by four kinds of methods to detect the positive rate of syphilis; syphilis screening test and control trial of each phase syphilis test results were compared. Results The positive rate of TRUST latency and tertiary syphilis was lower than that of the other three methods. There was no significant difference in the positive rate of syphilis between the first and second stages (P> 0.05) and the negative rate of recurrence after the cure (P <0.01). The positive rate of syphilis in the latent period of ELISA was lower than that of colloidal gold method and TPPA, but the difference was not statistically significant (all P> 0.05), and the positive rates of the other phases were the same as the other two methods. The positive rates of colloidal gold method and primary syphilis were slightly lower than that of TPPA and the positive rates of tertiary syphilis were lower than those of ELISA and TPPA (all P> 0.05) There was no significant difference between the two groups (P> 0.05). Conclusion TRUST, ELISA and colloidal gold method combined screening of syphilis can get good results, combined with history and clinical symptoms, but also to monitor the duration and efficacy.
