目的了解石家庄市肥胖人群含钾通道四聚化结构域15(KCTD15)基因rs11084753位点多态性情况,探讨其与白介素17(IL-17)的相关性和意义。方法应用2012-2014年河北省人民医院体检中心体检石家庄人群资料,选取肥胖组3 518例,同时选取正常人群3 500例为对照组。采用直接测序法检测KCTD15基因多态性,同时Elisa法检测血清中IL-17及血脂水平。结果肥胖组中TC、TG、IL-17在血清中水平与对照组比较明显升高,而肥胖组血清的HDL-C水平则比对照组明显降低(均P<0.05)。肥胖组中饮酒者的比例明显高于对照组,有阳性家族史的比例也明显高于对照组(均P<0.05)。根据KCTD15基因rs11084753位点多态性,肥胖组与对照组按AA、AG、GG不同基因型,TC、TG、IL-17水平依次升高,而HDL-C水平依次降低(均P<0.05)。基因多态性分析显示,肥胖人群KCTD15基因rs11084753位点GG及AG基因型明显高于对照组(P<0.05);多元Logistic回归分析显示,基因型和BMI是IL-17的独立影响因素(P<0.05)。结论石家庄市肥胖人群IL-17浓度及KCTD15基因rs11084753突变率明显高于对照组,KCTD15基因rs11084753位点突变可能影响血清IL-17的水平。
Objective To investigate the rs11084753 polymorphism of KCTD15 gene in obese people in Shijiazhuang and to explore its relationship with interleukin 17 (IL-17). Methods According to the physical examination data of Shijiazhuang from Hebei Provincial People’s Hospital during 2012-2014, 3 518 obesity patients were selected, and 3 500 healthy volunteers were selected as the control group. The KCTD15 gene polymorphism was detected by direct sequencing and Elisa assay was used to detect serum IL-17 and blood lipid levels. Results The levels of serum TC, TG and IL-17 in obesity group were significantly higher than those in control group, while the levels of HDL-C in obesity group were significantly lower than those in control group (all P <0.05). The proportion of drinkers in the obese group was significantly higher than that of the control group, and the positive family history was significantly higher than that of the control group (all P <0.05). According to the rs11084753 polymorphism of KCTD15 gene, the levels of TC, TG and IL-17 in obesity group and control group increased in turn according to AA, AG, GG genotypes, while HDL-C levels decreased in turn (all P <0.05) . Genetic polymorphism analysis showed that GG and AG genotypes of KCTD15 gene at rs11084753 in obese subjects were significantly higher than those in control subjects (P <0.05). Multivariate logistic regression analysis showed that genotype and BMI were independent factors of IL-17 (P <0.05). Conclusion The IL-17 concentration and the mutation rate of rs11084753 of KCTD15 gene in obese people in Shijiazhuang are obviously higher than those in control group. The mutation of rs1084753 in KCTD15 gene may affect the level of serum IL-17.
