目的 通过检测和分析杭州地区0~3岁健康儿童体内微量元素含量,初步建立参考区间,进而为该年龄段儿童合理控制体内微量元素含量水平提供参考和依据.方法 选取2016年1-12月来杭州市妇产科医院进行体检的0~3岁健康儿童1 491例(男812例,女679例),采用原子吸收光谱法分别测定全血中钙、铁、铜、锌、铅5种微量元素的含量.结果 健康男童与女童体内5种元素含量差异无统计学意义(P>0.05),可建立同一参考区间.铁、铜、锌、铅元素含量总体呈偏态分布,按百分位数P2.5及P97.5确定95%参考区间;钙元素含量总体呈正态分布,按-x±1.96s确定95%参考区间.研究结果显示,杭州地区健康儿童钙、铁、铜、锌、铅参考区间分别为1.49~1.93 mmol/L、7.17 ~9.49 mmol/L、12.53~29.80 μmol/L、52.30~91.35 μmol/L、21 ~59 μg/L,其中钙、铁、锌与国内其他地区相比差异有统计学意义(P<0.05),铅与厂家提供的参考区间相比更加灵敏,而铜与其他地区相比差异无统计学意义(P>0.05).结论 不同地区实验室应根据本地区实际情况确定参考区间,对指导本地区儿童完善饮食结构、合理补充体内微量元素及保障儿童身心健康发展具有重要意义.“,”Objective To detect and analyze the levels of trace elements in healthy children aged 0-3 years old in Hangzhou,preliminarily establish reference intervals,and further provide reference and basis for reasonably controlling the levels of trace elements in 0-3-year-old children.Methods A total of 1 491 healthy children aged 0-3 years old receiving physical examination in Hangzhou Obstetrics and Gynecology Hospital from January to December in 2016 were selected,812 boys and 679 girls were included.Atomic absorption spectrophotometry (AAS) was applied to detect the levels of calcium,iron,copper,zinc,and plumbum in whole blood.Results There was no statistically significant difference in the levels of calcium,iron,copper,zinc,and plumbum between healthy boys and healthy girls (P>0.05),so the same reference intervals could be established.The levels of iron,copper,zinc,and plumbum were skew distribution,95% reference intervals were determined according to P2.5 and P97.5.The distribution of calcium levels was normal,95% reference interval was determined according to (-x± 1.96s).The reference intervals of calcium,iron,copper,zinc,and plumbum in healthy children aged 0-3 years old in Hangzhou were 1.49-1.93 mmoL/L,7.17-9.49 mmol/L,12.53-29.80 μmol/L,52.30-91.35 μmol/L,and 21-59 μg/L,respectively.There were statistically significant differences in reference intervals of calcium,iron,and zinc between the children in Hangzhou and the children in other areas (P<0.05);compared with the reference interval provided by the manufacturer,the reference interval of plumbum was more sensitive,but there was no statistically significant difference in the reference interval of copper between the children in Hangzhou and the children in other areas (P>0.05).Conclusion The reference intervals should be determined according to the actual situation of the local area,which is of great significance to guide the children in the region to improve the dietary structure,reasonablely supplement trace elements,and ensure physical and mental health of the children.
