目的探究胃癌患者术后化疗前后外周血细胞角蛋白20(CK20 mRNA)的水平用作评估化疗效果的临床意义。方法对衡水市第二人民医院收治的29例胃癌患者术后进行化疗辅助治疗(FOLFOX4),分别在化疗前后的各时点收集患者的外周血,采用荧光定量PCR(RT-PCR)检测外周血中CK20 mRNA表达。对照组为18例健康志愿者。结果于化疗前一周进行检测,选取胃癌患者中有23例(79.3%)表达CK20 mRNA,6例(20.7%)不表达;有15例(51.7%)的CK20 mRNA表达升高,8例(27.6%)降低;CK20 mRNA表达量2.57±3.12。开始化疗时检测CK20 mRNA表达量3.12±1.98,与化疗前一周比较差异显著(P<0.05)。于化疗1周期后检测,20例(72.4%)患者表达CK20 mRNA,表达量1.89±1.43,与开始化疗时检测值比较有明显差异(P<0.05)。结论经过化疗胃癌患者术后外周血中CK20 mRNA表达降低,提示通过检测化疗前后外周血中CK20 mRNA表达可以预测胃癌患者术后的化疗效果。
Objective To investigate the clinical value of CK20 mRNA level in peripheral blood of patients with gastric cancer before and after chemotherapy. Methods Twenty-nine patients with gastric cancer admitted to Second People’s Hospital of Hengshui City underwent adjuvant chemotherapy (FOLFOX4). Peripheral blood samples were collected before and after chemotherapy, and peripheral blood was detected by real-time quantitative PCR (RT-PCR) CK20 mRNA expression. The control group was 18 healthy volunteers. RESULTS: CK20 mRNA was expressed in 23 cases (79.3%) of gastric cancer patients in one week before chemotherapy, 6 cases (20.7%) did not express CK20 mRNA, and 15 cases (51.7%) had CK20 mRNA expression in 8 cases (27.6% %) Decreased; CK20 mRNA expression was 2.57 ± 3.12. CK20 mRNA expression was 3.12 ± 1.98 at the beginning of chemotherapy, which was significantly different from that of the week before chemotherapy (P <0.05). After one cycle of chemotherapy, CK20 mRNA was expressed in 20 patients (72.4%), the expression was 1.89 ± 1.43, which was significantly different from the value of chemotherapy (P <0.05). Conclusion The decreased expression of CK20 mRNA in peripheral blood of patients with gastric cancer after chemotherapy suggests that the chemotherapeutic effect of gastric cancer patients can be predicted by detecting the expression of CK20 mRNA in peripheral blood before and after chemotherapy.
