目的探讨单纯性肥胖者血清内脏Vaspin(脂肪特异性丝氨酸蛋白酶抑制剂)水平与胰岛素抵抗(IR)的关系。方法采用ELISA法检测单纯性肥胖组(60例)和正常对照组(27例)的空腹血清vaspin水平,同时测定其空腹血糖(FBG)、胰岛素(FINS)、糖化血红蛋白(HbA1c)、血脂谱,计算体重指数(BMI)和腰臀比(WHR),并以稳态模型计算胰岛素抵抗指数(HOMA-IR)。结果单纯性肥胖组的血清vaspin水平显著高于正常对照组(P<0.01)。Vaspin与性别、BMI、腰围(W)、WHR、FINS、HOMA-IR及甘油三酯(TG)呈显著正相关(P<0.05),与高密度脂蛋白胆固醇(HDL-C)呈显著负相关(P<0.05);WHR、性别和HDL-C是血清vaspin的独立相关因素。结论单纯性肥胖者血清vaspin水平明显升高。Vaspin可能代偿性地参与了肥胖者IR的发生。
Objective To investigate the relationship between serum visceral Vaspin (fat-specific serine protease inhibitor) and insulin resistance (IR) in simple obesity. Methods Fasting serum vaspin levels in simple obesity group (n = 60) and normal control group (n = 27) were determined by ELISA. The levels of fasting blood glucose (FBG), insulin (FINS), HbA1c, Body mass index (BMI) and waist-hip ratio (WHR) were calculated and insulin resistance index (HOMA-IR) was calculated using a steady-state model. Results The serum vaspin level in simple obesity group was significantly higher than that in normal control group (P <0.01). There was a significant positive correlation between Vaspin and sex, BMI, WHR, WHR, FINS, HOMA-IR and triglyceride (TG) and negatively correlated with high density lipoprotein cholesterol (HDL-C) (P <0.05). WHR, sex and HDL-C were independent correlates of serum vaspin. Conclusion The serum vaspin level of simple obesity patients was significantly higher. Vaspin may be compensatory participation in the occurrence of obesity IR.
