目的对OLYMPUS-PK7300全自动血型检测系统在无偿献血者血液检测中的应用评价。方法凝集法采用OLYMPUS-PK7300全自动血型分析仪对本站2015年1-6月50 461例无偿献血者标本进行ABO血型及Rh D血型检测;判读失败标本采用手工试管法定型,疑难血型送中心实验室进一步确认。结果 OLYMPUS-PK7300全自动血型分析仪判读失败849例,占1.68%(849/50 461),经分析,其中反定型判读错误590例(69.49%),1次性判读成功率98.32(49 612/50 461),共检出Rh D阴性485例(485/50 461),经中心实验室确证Rh D阴性475例,Rh弱D型10例。对849例判读失败标本经手工试管法纠正831例(831/849),18(18/849)例标本正反定型不符,送中心实验室确证12例(12/18)正反定型相符,4例(4/18)为亚型,2例(2/18)为不规则抗体。A、B、O、AB 4种血型间判读失败率差异有显著统计学意义(χ~2=216.33,P<0.01),其中A型判读失败率较高。结论 OLYMPUS-PK7300全自动血型检测系统大大提高了工作质量和效率,适用于无偿献血者大批量血型筛查工作,但多种因素可能造成误判,要针对不同原因采取纠正措施,与手工试管法联合,有效保证临床输血安全。
Objective To evaluate the application of OLYMPUS-PK7300 automatic blood type detection system in the blood test of unpaid blood donors. Methods The OLYMPUS-PK7300 automatic blood analyzer was used to detect the blood samples of 50 461 unpaid blood donors from January 2015 to June 2015 in ABO blood group and Rh D blood group. The unsuccessful interpretations were performed by manual test tube method and difficult blood donation center Laboratory confirmed further. Results The OLYMPUS-PK7300 automatic blood analyzer failed to interpret 849 cases, accounting for 1.68% (849/50 461). Among them, 590 cases (69.49%) were negative interpretive errors and 98.32% (49 612) / 50 461). A total of 485 Rh-negative cases (485/50 461) were detected. Rh-negative 475 cases and Rh weak-D 10 cases were confirmed by central laboratory. 849 cases (831/849) and 18 (18/849) cases were corrected by manual test-tube method. There were 12 positive (12/18) Cases (4/18) were subtypes, 2 cases (2/18) were irregular antibodies. There was a significant difference in the failure rate of interpretation between A, B, O and AB (χ ~ 2 = 216.33, P <0.01), and the failure rate of type A interpretation was higher. Conclusion OLYMPUS-PK7300 automatic blood type detection system greatly improves the quality of work and efficiency, suitable for large-scale blood donation blood screening, but many factors may cause miscarriage of justice, to take corrective measures for different reasons, and manual test tube method Joint, effectively ensure the safety of clinical transfusion.
