目的探讨超声引导下经皮集束电极射频消融治疗肾肿瘤的可行性和有效性。方法将42只已建立肾VX2肿瘤模型的大白兔随机分为对照组15只和治疗组27只,对治疗组行超声引导下射频经皮消融肿瘤结节,对照组未行处理,应用超声显像技术分别于治疗前、治疗中和治疗后按不同时间点观察两组肿瘤大小、回声及形态,至兔自然死亡时比较肿瘤生长和肺转移情况,计算生存率。结果①27只兔均经超声引导穿刺成功,射频治疗时间27~220s;②射频治疗后3d、7d、14d、21d肿瘤平均直径均小于对照组(P<0.05),治疗组肿瘤生长较对照组明显缓慢;③治疗组与对照组荷瘤生存时间差异有显著性意义[(45.1±9.5)d对(33.7±6.9)d,P<0.05];④同一时间点治疗组荷瘤肾明显小于对照组,肺转移结节数治疗组少于对照组。结论超声引导下集束电极射频消融肾肿瘤可延长生存期,操作简便,安全、有效。
Objective To investigate the feasibility and effectiveness of ultrasound-guided radiofrequency ablation of renal tumors in percutaneous bundled electrodes. Methods Forty-two rabbits with established renal VX2 tumor model were randomly divided into control group (n = 15) and treatment group (n = 27). Ultrasound-guided radiofrequency ablation of the tumor nodules was performed in the treatment group. The tumor size, echogenicity and morphology of the two groups were observed before treatment, during treatment and after treatment, respectively. The tumor growth and lung metastasis were compared with the natural death of rabbits to calculate the survival rate. Results ①Twenty rabbits were successfully guided by ultrasound and the duration of radiofrequency ablation was 27 ~ 220s. ②The average tumor diameter of the tumors at 3d, 7d, 14d and 21d after radiofrequency ablation were all less than those in the control group (P <0.05) (45.1 ± 9.5) d vs (33.7 ± 6.9) d, P <0.05]; ④The tumor-bearing kidney in the treated group at the same time point was significantly lower than that in the control group , The number of pulmonary metastases in the treatment group than in the control group. Conclusion Ultrasound guided bundled electrode radiofrequency ablation of renal tumors can prolong survival, easy to operate, safe and effective.
