代谢性酸中毒一般是根据阴离子隙(AG)是否升高而分为两类。总的来说,酸产生过多时AG增高,高氯性酸中毒时AG正常。最近我们证实了一种由于甲苯氧化引起马尿酸生成过多所致的代谢性酸中毒,其AG接近正常。而且,代谢性酸中毒伴有AG增高者并非基于产酸过多(如肾功能不全),因此,我们提出了一个主要基于病理生理而不是单纯依靠临床生化检查的分类方法,这一新方法的根据是酸的产生是否过多。
Metabolic acidosis is generally divided into two groups based on whether the anion gap (AG) is elevated. In general, AG is increased when too much acid is produced and AG is normal when highly chlorinated acid is poisoned.
