目的分析珠海市艾滋病(AIDS)病人接受国家免费抗病毒治疗(ART)前的基本特征,为探讨治疗工作模式机制提供依据。方法利用国家艾滋病综合防治信息系统,收集2005年1月1日至2014年9月30日,成人艾滋病抗病毒治疗病人的基本情况,建立Excel数据库,并使用SPSS 18.0软件进行分析。结果592例病人中76.2%(451例)为男性,平均年龄为(36.2±10.8)岁,68.7%(407例)经性传播,男男性行为传播病人比例逐年增高(χ2趋势=52.115,P<0.001)。55.7%的病人(330例)开始治疗时已进入艾滋病临床Ⅳ期。579例病人的基线CD+4T淋巴细胞(简称CD4细胞)计数中位值为177个/μL。2011年国家艾滋病抗病毒治疗标准调整后,进入ART的AIDS病人处于早期临床阶段(Ⅰ/Ⅱ期)的比例显著高于标准调整前(χ2=94.281,P<0.0001),基线CD4细胞计数也明显上升(t=14.4,P<0.0001)。结论晚发现和晚治疗是当地AIDS病人ART前的突出特点,需探讨更有效的工作机制,扩大监测检测网络,优化从筛检到治疗的工作程序。
Objective To analyze the basic characteristics of AIDS patients before receiving the national free anti-virus treatment (ART) in Zhuhai, and to provide evidence for the mechanism of treatment work. Methods The National AIDS Integrated Prevention and Control Information System was used to collect the basic information of adult AIDS patients treated with antiviral therapy from January 1, 2005 to September 30, 2014. An Excel database was established and analyzed by using SPSS 18.0 software. Results Among the 592 patients, 76.2% (451 cases) were male and the average age was (36.2 ± 10.8) years old. 68.7% (407 cases) were sexually transmitted and the proportion of MSM transmission increased year by year (χ2 trend = 52.115, P < 0.001). 55.7% of the patients (330 cases) have already started to enter stage IV of AIDS when they started treatment. The median CD + 4T lymphocyte count (CD4) count for 579 patients was 177 cells / μL. After adjusting for the national HIV / AIDS treatment standard in 2011, the proportion of AIDS patients entering ART in the early clinical stage (stage Ⅰ / Ⅱ) was significantly higher than that before standard adjustment (χ2 = 94.281, P <0.0001), and the baseline CD4 cell count was also significant Increased (t = 14.4, P <0.0001). Conclusion Late detection and late treatment are the prominent features of local AIDS patients before ART, more effective working mechanisms need to be explored, the network of monitoring and detection expanded, and the working procedure from screening to treatment should be optimized.
