目的探讨糖尿病慢性肾脏疾病(CKD)患者血清骨钙素(OC)与视黄醇结合蛋白4(RBP4)及胱抑素C(Cys-C)水平变化的关系。方法 选取120例CKD患者,根据eGFR分为eGFR≥90ml/(min·1.73 m~2)组36例,60≤eGFR<90 ml/(min·1.73 m~2)组60例,eGFR<60 ml/(min·1.73 m~2)组24例,另选取健康体检者45名作为对照组(NC)。各组均测生化指标及血清OC、RBP4、Cys-C水平。结果 eGFR≥90 ml/(min·1.73 m~2)组OC低于NC组及eGFR<60 ml/(min·1.73 m~2)组(P<0.05);60≤eGFR<90 ml/(min·1.73 m~2)组、eGFR<60 ml/(min·1.73 m~2)组RBP4高于其他两组(P<0.05);eGFR<60 ml/(min·1.73 m~2)组Cys-C高于其他组(P<0.05)。相关性分析显示,OC、RBP4、Cys-C与eGFR呈负相关(r=-0.172、-0.304、-0.348,P<0.01),OC与RBP4、Cys-C呈正相关(r=0.133、0.206,P<0.05)。结论 OC、RBP4及Cys-C可作为评价CKD早期肾损伤及骨代谢异常的有效指标。
Objective To investigate the relationship between serum osteocalcin (OC) and retinol binding protein 4 (RBP4) and cystatin C (Cys-C) levels in patients with chronic kidney disease (CKD). Methods A total of 120 patients with CKD were enrolled and divided into eGFR≥90ml / (min · 1.73m ~ 2) group and eGFR <60ml group with 60≤eGFR <90ml / (min · 1.73m ~ 2) /(min.l.73 m ~ 2) group, 24 cases, another 45 healthy subjects were selected as the control group (NC). The biochemical indexes and the levels of serum OC, RBP4, Cys-C in each group were measured. Results Compared with NC group and eGFR <60 ml / (min · 1.73 m ~ 2) group, eGFR≥90 ml / (min · 1.73 m ~ 2) RBF4 was significantly higher in eGFR <60 ml / (min · 1.73 m ~ 2) group than in other two groups (P <0.05); eGFR <60 ml / (min · 1.73 m ~ C was higher than other groups (P <0.05). Correlation analysis showed that OC, RBP4 and Cys-C were negatively correlated with eGFR (r = -0.172, -0.304, -0.348, P <0.01), and OC was positively correlated with RBP4 and Cys- P <0.05). Conclusion OC, RBP4 and Cys-C can be used as an effective index to evaluate the early renal damage and abnormal bone metabolism in CKD.
