本研究对15例小脑髓母细胞瘤外科切除标本作了超微结构和10种抗体(P53、PCNA、NF、NSE、GFAP、S-100、Vi、LCA、CK、NF)免疫组化观察,描述了典型髓母细胞瘤和促纤维增生型髓母细胞瘤的一般形态,超微结构和免疫组化特点。发现:超微结构和免疫组化均支持瘤细胞主要朝向神经细胞分化,同时有残存的胶质细胞及有髓纤维。PCNA在15例肿瘤中100%阳性表达,阳性细胞百分率更高,平均39.40%,说明该瘤增殖活性强、分化差。P53蛋白阳性率较以往观察的高,15例中有11例表达阳性,占73%,且细胞阳性百分率高,平均33.3%,说明P53基因突变在该瘤是存在的,与肿瘤的发生有关。讨论中还总结了髓母细胞瘤的诊断要点。
In this study, immunohistochemical observation of ultrastructure and 10 antibodies (P53, PCNA, NF, NSE, GFAP, S-100, Vi, LCA, CK, NF) was performed in 15 cases of cerebellar medulloblastoma. Described the typical medulloblastoma and fibroblastic proliferative medulloblastoma morphology, ultrastructure and immunohistochemical characteristics. It was found that both ultrastructure and immunohistochemistry supported the tumor cells to differentiate mainly toward neurons, with residual glial cells and myelinated fibers. PCNA in 100 cases of 15 cases of positive expression of positive cells, a higher percentage of positive cells, an average of 39.40%, indicating that the tumor proliferation and poor differentiation. The positive rate of P53 protein was higher than that observed in the past, and 11 of 15 cases were positive, accounting for 73%. The positive percentage of P53 protein was 33.3%, which indicated that P53 gene mutation existed in this tumor, related. The discussion also summarized the main points of diagnosis of medulloblastoma.
