[目的]探讨KDM4A在儿童和成人甲状腺乳头状癌中的表达及诊断价值。[方法]采用实时荧光定量PCR的方法进行组织KDM4A表达检测,其中甲状腺乳头状癌组织样本22例,癌旁组织样本13例(配对样本共13对)。其中,儿童甲状腺乳头状癌样本10例,癌旁组织3例(配对样本3对);成人甲状腺乳头状癌标本12例,癌旁组织10例(配对样本10对)。并采用受试者工作特征(ROC)曲线对其诊断价值进行评估。[结果]甲状腺乳头状癌组织较配对癌旁组织中KDM4A表达显著增高(P=0.011),非配对比较甲状腺乳头状癌组织较癌旁组织中KDM4A表达显著增高(P=0.027);KDM4A诊断PTC的ROC曲线下面积指数为0.794(P=0.004)(Youden指数0.538);儿童甲状腺乳头状癌中KDM4A表达略高于成人(P>0.05),且儿童甲状腺乳头状癌标本中KDM4A的ROC曲线下面积指数为0.831(P=0.007)(Youden指数0.615),高于成人标本的0.763,P=0.026(Youden指数0.538)。[结论 ]KDM4A表达在甲状腺乳头状癌中显著高于癌旁组织,并明确了KDM4A具有诊断价值。此外,本研究还明确了KDM4A作为分子标志物在儿童甲状腺乳头状癌中的诊断价值高于成人。
[Objective] To investigate the expression and diagnostic value of KDM4A in papillary thyroid carcinoma in children and adults. [Methods] Tissue KDM4A expression was detected by real-time fluorescence quantitative PCR, including 22 cases of thyroid papillary carcinoma tissue samples and 13 cases of paracancerous tissue samples (paired samples of 13 pairs). Among them, 10 cases of papillary thyroid carcinoma in children and 3 cases of paracancerous tissues (3 paired samples); 12 cases of papillary thyroid carcinoma in adults and 10 cases of paracancerous tissues (10 paired samples). The receiver operating characteristic (ROC) curve was used to evaluate the diagnostic value. [Results] The expression of KDM4A in papillary thyroid carcinoma tissues was significantly higher than that in paired paracancer tissues (P = 0.011). The expression of KDM4A in paired thyroid papillary carcinoma tissues was significantly higher than that in paracancerous tissues (P = 0.027) The area under the ROC curve was 0.794 (P = 0.004) (Youden index 0.538). The expression of KDM4A in children with thyroid papillary carcinoma was slightly higher than that in adults (P> 0.05). The ROC curve of KDM4A in children with papillary thyroid carcinoma The area index was 0.831 (P = 0.007) (Youden index 0.615), higher than the 0.763 for adult samples, P = 0.026 (Youden index 0.538). [Conclusion] The expression of KDM4A in papillary thyroid carcinoma was significantly higher than that in paracancerous tissues, and the diagnostic value of KDM4A was clear. In addition, this study also made clear that KDM4A as a molecular marker in the diagnosis of papillary thyroid carcinoma in children than in adults.
