目的:探讨降钙素原(PCT)、C反应蛋白(CRP)、中性粒细胞(CD64)、干扰素诱导蛋白10(IP-10)联合检测在儿童上呼吸道感染性疾病诊断中的应用价值。方法:选取2017年1月至2018年12月绍兴文理学院附属医院收治的上呼吸道感染性疾病患儿122例为研究对象,根据患儿临床症状及诊断项目结果,患儿均为细菌感染(感染组),另选同期体检健康儿童作为对照组(n n=61),分别对两组受试者PCT、CRP、CD64、IP-10水平进行检测,同时对各指标之间相关性以及联合检测的诊断价值进行分析。n 结果:对照组和感染组PCT分别为(0.55±0.13)μg/L、(10.81±1.25)μg/L,CRP分别为(1.85±1.07)mg/L、(15.18±5.05)mg/L,CD64指数分别为(4.37±0.51)、(9.91±1.62),IP-10分别为(0.08±0.01)μg/L、(0.70±0.20)μg/L,差异均有统计学意义(n t=63.847、20.362、26.040、24.163,均n P<0.001)。与对照组比较,感染组的PCT、CRP、CD64、IP-10均显著升高(均n P<0.05);治疗后,感染组PCT、CRP、CD64、IP-10水平均下降(均n P<0.05);经Pearson相关性分析发现,PCT与CRP、PCT与CD64、PCT与IP-10、CRP与CD64、CRP与IP-10、CD64与IP-10之间均呈正相关(均n P<0.05);采用ROC曲线进行细菌感染性诊断价值分析,PCT、CRP、CD64、IP-10联合诊断的灵敏度为98.36%、特异度为97.52%,均高于单指标。n 结论:PCT、CRP、CD64、IP-10联合检测有较高的敏感性、特异性,可作为诊断儿童上呼吸道感染性疾病早期细菌感染的指标。“,”Objective:To explore the clinical value of combined detection of procalcitonin (PCT), C-reactive protein (CRP), neutrophils (CD64), interferon-induced protein 10(IP-10) in the diagnosis of upper respiratory infectious diseases in children.Methods:A total of 122 children with upper respiratory tract infectious diseases treated in the Affiliated Hospital of Shaoxing University of Arts and Sciences from January 2017 to December 2018 were selected as the research subjects.According to the clinical symptoms and diagnosis results of the children, the children were all infected by bacteria (infected group), and healthy children in the same period were selected as normal control group (n n=61). The PCT, CRP, CD64 and IP-10 levels of the two groups were detected.The correlation between the indicators and the diagnostic value of combined detection was analyzed.n Results:The PCT levels of the normal control group and infected group were (0.55±0.13)μg/L, (10.81±1.25)μg/L, respectively, and the CRP levels of the normal control group and infected group were (1.85±1.07)mg/L, (15.18±5.05)mg/L, respectively, and the CD64 index of the normal control group and infected group were (4.37±0.51), (9.91±1.62), respectively, and the IP-10 levels of the normal control group and infected group were (0.08±0.01)μg/L, (0.70±0.20)μg/L, respectively, the differences were statistically significant between the two groups(n t=63.847, 20.362, 26.040, 24.163, all n P<0.001). The levels of PCT, CRP, CD64 and IP-10 in the infection group were higher than those in the normal control group.After treatment, the levels of PCT, CRP, CD64 and IP-10 in the infection group were decreased (alln P<0.05). Pearson correlation analysis found that PCT and CRP, PCT and CD64, PCT and IP-10, CRP and CD64, CRP and IP-10, CD64 and IP-10 were positively correlated (alln P<0.05). The sensitivity, specificity and Yoden index of combined diagnosis were higher than those of single index by ROC.n Conclusion:The combined detection of PCT, CRP, CD64 and IP-10 has high sensitivity and specificity.It can be used as an index for the diagnosis of early bacterial infection in children with upper respiratory infectious diseases.
