报道2例由于支扩或胸部外伤性大咯血所致一侧全肺不张引起的急性呼衰,是临床并非鲜见,处理棘手之急症,若不及时处理常危及患者生命,我院采用高频呼吸机与纤支镜有机结合,把握时机,密切监测血气变化,合理调整高频通气参数,通过纤支镜直接向气管内高频喷射通气,能在有效氧供的条件下利用纤支镜消除支气管堵塞并行局部止血,病人呼衰得以迅速纠正,打破了急性咯血期不宜纤支镜手术传统观念,为今后此类病人的抢救开劈了新的途径。
