目的 研究肺癌组织中CD44和TSP 1的表达并分析其与患者临床预后的关系。方法 应用免疫组化法检测 112例肺癌组织中CD44和TSP 1表达 ,其中 2 5例标本同时用RT PCR法检测TSP 1表达。结果 TSP 1在肺腺癌中的表达 (74.36 % )明显高于鳞癌和小细胞癌 (47.17% ,P =0 .0 3 ;35 .71% ,P =0 .0 2 )。CD44H及CD44V6 在小细胞肺癌中均表达极低 ;CD44H 在肺腺癌中的表达 (6 1.5 4% )高于鳞癌(5 0 .94% ,P =0 .0 36 ) ,CD44V6 在肺鳞癌中的表达 (5 0 .94% )明显高于腺癌 (17.95 % ,P =0 .0 0 3)。Kaplan Meier生存分析发现 ,TSP 1、CD44H 及CD44V6 均与肺癌患者生存情况无明显关系 ,但以病理类型进行分层分析后 ,在肺腺癌的表达中 ,低TSP 1表达组的生存率明显高于中 /高表达组 (LogRank =0 .0 16 3)。 2 5例标本同时用免疫组化法及RT PCR法检测TSP 1表达 ,两法符合率为 84% (2 1/ 2 5 ) ,两法结果无显著性差异 (P >0 .0 5 )。结论 肺癌的TSP 1、CD44H 及CD44V6 表达均有一定的病理学特异性。肺腺癌中TSP 1可能与患者的预后有关。TSP 1的免疫组化检测法能可靠地用于石蜡存档标本的检测
Objective To study the expression of CD44 and TSP 1 in lung cancer tissues and analyze their relationship with clinical prognosis. Methods Immunohistochemical method was used to detect the expression of CD44 and TSP 1 in 112 cases of lung cancer. Twenty-five specimens were simultaneously examined for the expression of TSP 1 by RT PCR. Results The expression of TSP 1 in lung adenocarcinoma (74.36 %) was significantly higher than that of squamous cell carcinoma and small cell carcinoma (47.17%, P =0.03; 35.71%, P =0.02). Both CD44H and CD44V6 were extremely low in small cell lung cancer; the expression of CD44H in lung adenocarcinoma (6 1.5 4%) was higher than that in squamous cell carcinoma (50.94%, P =0.036), and CD44V6 was in the lung scale. The expression in cancer (5.94%) was significantly higher than that in adenocarcinoma (17.95%, P = 0.03). Kaplan Meier survival analysis found that TSP 1, CD44H, and CD44V6 were not significantly associated with the survival of patients with lung cancer, but after stratified analysis by pathological type, the survival rate of low TSP 1 expression group was significantly higher in the expression of lung adenocarcinoma. In the medium/high expression group (LogRank =0. 0 16 3). Twenty-five specimens were examined for both expressions of TSP 1 by immunohistochemistry and RT PCR. The coincidence rate of the two methods was 84% (2 1/25). There was no significant difference between the two methods (P > 0.05). Conclusion The expression of TSP 1, CD44H and CD44V6 in lung cancer are pathologically specific. TSP 1 in lung adenocarcinoma may be related to the prognosis of the patient. TSP 1 immunohistochemical detection can be reliably used for the detection of paraffin-embedded specimens
