目的:总结显微血管减压术(MVD)治疗三叉神经痛(TN)的经验。方法:MVD治疗TN,术中显露面三叉神经全程及入脑干区(REZ),明确责任血管后将其推移,在血管与脑干之间放置Teflon棉。结果:术中见三叉神经入脑干处(REZ)有血管压迫53例(94.6%)。MVD术后总有效率为91.1%。结论:术中三叉神经全程及其REZ区的充分显露,责任血管的识别,三叉神经REZ的充分减压,术中岩静脉处理、“零牵拉”技术均是影响疗效的重要因素。
Objective: To summarize the experience of microvascular decompression (MVD) in the treatment of trigeminal neuralgia (TN). Methods: MVD was used to treat TN. The entire surface of the trigeminal nerve and REZ were exposed intraoperatively. The vessel was definitively responsible for its displacement and Teflon cotton was placed between the blood vessel and the brainstem. Results: During operation, 53 cases (94.6%) had vascular compression in the trigeminal nerve into the brain stem (REZ). The total effective rate after MVD was 91.1%. CONCLUSION: Full exposure of the trigeminal nerve throughout the operation and its REZ zone, identification of responsible vessels, adequate decompression of REZ in the trigeminal nerve, intraoperative petroclival treatment and “zero-pull” techniques are all important factors that influence curative effect.
