目的:用多层螺旋CT扫描观察肺密度与慢性阻塞性肺疾病(COPD)患者肺功能的关系。方法:选取2011年10月~2012年2月在我院呼吸科就诊的慢性阻塞性肺疾病患者87例,设为观察组,同时选择同时期的非肺部疾病的患者85例为对照组。所有患者均行多层螺旋CT检查(64层)与肺功能检查,对比肺密度值与FEV1和FEV1/FVC值的关系以及和COPD严重程度分级的关系。结果:比较2组患者各项肺密度值指标,结果观察组平均ALDin值为(-845±42)、平均ALDex值为(-789±53)、平均ALDin-ALDex值为(-56±24)、平均ALDex/ALDin值为(0.92±0.03)、平均(ALDin-ALDex)/ALDin的值为(-0.06±0.02),与对照组比较,差异显著;慢阻肺患者ALDin—ALDex、ALDex、(ALDin—ALDex)/ALDin、ALDex/ALDin等肺密度值与各项肺功能值有显著的差异性。以上均P<0.05。结论:多层螺旋CT能够准确并且便捷的得到肺功能的评价指标,与PFtr检查相关性较好,肺密度值能够筛查及预测COPD患者,临床应用的价值较高。
Objective: To investigate the relationship between lung density and lung function in patients with chronic obstructive pulmonary disease (COPD) by multi-slice spiral CT. Methods: From October 2011 to February 2012, 87 patients with chronic obstructive pulmonary disease (COPD) admitted to our department were selected as the observation group and 85 patients with non-pulmonary disease during the same period were selected as the control group. All patients underwent multislice CT examination (64 levels) and pulmonary function tests, and the relationship between lung density and FEV1 and FEV1 / FVC values and the relationship with severity of COPD were compared. Results: The lung density index of the two groups were compared. The average ALDin value in the observation group was (-845 ± 42), the average ALDex value was -789 ± 53, and the average ALDin-ALDex value was -56 ± 24 (ALDin-ALDex) / ALDin was (-0.06 ± 0.02), which was significantly different from the control group. The levels of ALDin-ALDex, ALDex, ALDin-ALDex) / ALDin, ALDex / ALDin and other pulmonary density values and the lung function values were significantly different. All above P <0.05. Conclusion: MSCT can accurately and conveniently obtain the evaluation index of pulmonary function, which has good correlation with PFtr examination. The lung density value can be used in screening and predicting COPD patients, and the clinical value is higher.
