目的乌鲁木齐地区部分正常人群甲状腺过氧化物酶抗体(TPOAb)及甲状腺球蛋白抗体(Tg Ab)阳性临界值的确定。方法按美国国家临床生化协会(NACB)指南中关于甲状腺疾病诊断的筛选要求,2013年5月采用横断面方法随机抽取乌鲁木齐市两个社区常住居民进行流行病学调查,完善问卷调查,采集血液样本,测定TSH、TPOAb及Tg Ab的水平,同时行甲状腺B超检查,最终筛选出符合标准的男性168名。结果总人群的TPOAb高值95%可信区间上限为42.96IU/ml,Tg Ab高值95%可信区间上限为29.48IU/ml。其中汉族TPOAb高值95%可信区间上限为32.54IU/ml,维吾尔族TPOAb高值95%可信区间上限为59.84IU/ml,差异有统计学意义(P=0.020);汉族Tg Ab高值95%可信区间上限为21.84IU/ml,维吾尔族Tg Ab高值95%可信区间上限为43.51IU/ml,维吾尔族高于汉族,差异无统计学意义(P=0.490)。结论本研究认为当TPOAb为42.96IU/ml及Tg Ab为29.48 IU/ml时可定义为阳性临界值。
Objective To determine the positive cut-off value of thyroid peroxidase antibody (TPOAb) and thyroglobulin antibody (Tg Ab) in some normal population in Urumqi. Methods According to the NACB guidelines for thyroid disease screening in May 2013, a cross-sectional method was used to randomly select residents of two communities in Urumqi for epidemiological investigation, improve the questionnaire and collect blood samples . The levels of TSH, TPOAb and Tg Ab were measured. Thyroid B ultrasound examination was performed at the same time, and 168 males were finally screened out. Results The overall population had a 95% confidence interval upper limit of TPOAb of 42.96 IU / ml and a high 95% confidence interval of Tg Ab of 29.48 IU / ml. Among them, the upper limit of 95% confidence interval of TPOAb was 32.54 IU / ml in Han nationality, and the upper limit of 95% confidence interval of TPOAb was 59.84 IU / ml in Uighur ethnic group, with significant difference (P = 0.020) The upper limit of 95% confidence interval was 21.84 IU / ml, the upper limit of 95% confidence interval of Uygur Tg Ab was 43.51 IU / ml, and the Uygur nationality was higher than Han nationality (P = 0.490). Conclusions This study suggests that a positive cutoff may be defined when TPOAb is 42.96 IU / ml and Tg Ab is 29.48 IU / ml.
