目的分析交叉克氏针结合外固定架固定治疗儿童移位股骨颈骨折的疗效。方法回顾性分析22例儿童移位股骨颈骨折的临床资料,术中先行闭合复位,然后经皮置入3或4枚克氏针交叉固定股骨颈骨折,最后用外固定架固定。结果本组均获得平均26(8~60)个月随访,骨折均愈合,愈合时间6~20周,平均14周。出现股骨头缺血性坏死2例,钉道感染3例。按Ratliff标准评价疗效:优20例,可1例,差1例,优良率90.9%。结论交叉克氏针结合外固定架固定是治疗儿童股骨颈骨折的有效方法,具有手术操作简便、固定可靠、骨折愈合快、远期疗效满意的优点。
Objective To analyze the effect of cross Kirschner wire fixation with external fixator in the treatment of displaced femoral neck fracture in children. Methods The clinical data of 22 cases of displaced femoral neck fractures were retrospectively analyzed. The patients received closed reduction first and then 3 or 4 Kirschner wires were percutaneously fixed to fix the femoral neck fracture. Finally, the external fixator was used. Results All patients were followed up for an average of 26 (8 ~ 60) months. All the fractures healed. The healing time ranged from 6 to 20 weeks with an average of 14 weeks. 2 cases of avascular necrosis of the femoral head, 3 cases of nail infection. The curative effect was evaluated according to Ratliff criteria: excellent in 20 cases, fair in 1 case and poor in 1 case, excellent and good rate was 90.9%. Conclusion Cross Kirschner wire fixation with external fixators is an effective method for the treatment of femoral neck fractures in children. It has the advantages of simple operation, reliable fixation, fast fracture healing and long-term curative effect.
